Project Description: The Department of Health and Human Services, Division of Services for People with Disabilities seeks to provide time-limited peer support services to Persons and their families to help them remain in their home and community-based settings while waiting for Home and Community Based Services (HCBS). Peer support services will be provided by people with lived experience to help identify needs, provide support and guidance, navigate systems, and connect Persons and families to resources.
