The Department of Homeless Services (DHS) is seeking an appropriately qualified vendor to provide street outreach, case management and housing placement services to individuals experiencing homelessness in public spaces, including the street and subway, many of whom face complex challenges including serious mental illness and/or substance use disorders. Outreach teams are a referral source or service-rich placements designed to engage individuals who are not yet ready for traditional shelter, offering a pathway toward permanent housing. There will be 4 contracts for outreach within the MTA system, which will be broken down as follows: (1) Catchment Area 1- the MTA system within Manhattan, (2) Catchment Area 2- the MTA system within Bronx, (3) Catchment Area 3- the MTA system within the Brooklyn and (4) Catchment Area 4 the MTA system within Queens.  There will be one award per Competition Pool (Catchment Area). The contractor will have teams go into the MTA System, engage individuals experiencing unsheltered homelessness, provide assessments, services and placements, and case manage the clients through to permanent housing.
