1. INTRODUCTION
Purpose and Overview
This Request for Bids (RFB) is published to solicit bids to provide semi-annual
preventative maintenance and repairs and to provide emergency repair service for
overhead doors and motors located at Glencliff Home property, located at 393 High
Street, Glencliff, NH 03238.
The selected Bidder must provide all labor, equipment, materials, and tools necessary
to complete the terms set forth in Section 2. Scope of Work.
Contract Period
1.2.1. Contracted services are anticipated to begin on July 1, 2022, through June
30, 2024.
1.2.2. The Department reserves the right to renew the resulting contract for up
to four (4) additional years, subject to continued availability of funds,
satisfactory performance of services, agreement of the parties, and
approval by the Governor and Executive Council.
2. Scope of Work
The selected Bidder must provide semi-annual preventative maintenance or
repairs for fifteen (15) overhead doors and motors, or otherwise agreed upon
number of overhead doors and motors as agreed upon by the selected Bidder
and Department.
The selected Bidder must be able to provide, on a 24/7 basis, emergency repair
service that will commence within four (4) hours of notification by the Plant
Manager Engineer or their designee.
The selected Bidder must not charge a minimum hour requirement for their
services.
The selected Bidder must contact the Plant Maintenance Engineer or their
designee between the business hours of 7:00 a.m. and 3:00 p.m. to schedule
semi-annual preventative maintenance and repairs.
The Contractor shall provide the Plant Maintenance Engineer, or designee, with
the estimated time frame for completion of services and the number of
Contracted employees that will be needed to perform the scope of work.
The Contractor shall sign in and submit to screening for communicable
diseases, including screening for the COVID-19 virus, as required by the
Department upon arrival at the job site.
The Contractor shall report to the Plant Maintenance Engineer, or designee,
upon arrival to the job site and check with the appropriate staff for any identified
operational deficiencies.
The Contractor shall provide a written summary of work performed after each
scheduled or emergency call and obtain the signature of the Plant Maintenance

Engineer, or designee, before leaving the job site.
The selected Bidder must provide all necessary tools, man lift, equipment,
materials and labor to perform all tasks required.
The selected Bidder must conduct, at the request of the Department, semi-
annual cleaning, tune-ups, and preventative maintenance of overheard doors,
which includes, but is not limited to:
2.10.1. Inspecting door section condition.
2.10.2. Inspecting and adjusting door alignment.
2.10.3. Inspecting and lubricating rollers and bearings.
2.10.4. Adjusting and lubricating springs.
2.10.5. Inspecting springs for secure mounting.
2.10.6. Cleaning, inspecting and tightening hinges and hardware.
2.10.7. Inspecting cables for wear and damage.
2.10.8. Inspecting drums for wear and damage.
2.10.9. Inspecting locks for proper operations.
2.10.10. Inspecting weather stripping for wear and damage.
2.10.11. Cleaning, inspecting and lubricating tracks.
2.10.12. Inspecting track fasteners and hangers.
2.10.13. Inspecting and lubricating pulleys and chain hoist.
The selected Bidder must provide semi-annual cleaning, tune-ups, and
preventative maintenance of motors, which includes, but is not limited to:
2.11.1. Inspecting and adjusting limit switches.
2.11.2. Inspecting and adjust belts.
2.11.3. Inspecting and lubricating roller chains.
2.11.4. Inspecting and adjusting brakes.
2.11.5. Inspecting and adjusting clutches.
2.11.6. Inspecting operator mountings.
2.11.7. Inspecting and testing disconnects.
2.11.8. Inspecting and tightening of all sprockets.
2.11.9. Lubricating all bearings.
The selected Bidder must provide the Plant Maintenance Engineer or designee
a written estimate for any repair work that includes costs of all parts and labor
to complete the repair prior to beginning repair work.
The selected Bidder must obtain approval from the Plant Maintenance Engineer
or designee prior to initiating any repair services.

The selected Bidder must ensure:
2.14.1. All repair work is performed in accordance with the manufacturer’s
specifications.
2.14.2. All repair work is completed by a certified technician.
The selected Bidder must obtain, at the Contractor’s expense, a Criminal
Background Check for each staff person providing service at the Glencliff Home
and shall release the results to the Department’s Office of Human Resources
to ensure no convictions for the following crimes:
2.15.1. A felony for child abuse or neglect, spousal abuse and any crime
against children or adults, including but not limited to: child
pornography, rape, sexual assault, or homicide;
2.15.2. A violent or sexually-related crime against a child or adult, or a crime
which may indicate a person might be reasonably expected to pose a
threat to a child or adult; and
2.15.3. A felony for physical assault, battery, or a drug-related offense
committed within the past five(5) years in accordance with 42 USC 671
(a) (20) (A) (ii).
The selected Bidder must authorize the Department to conduct a Bureau of
Elderly and Adults Services (BEAS) State Registry check at no cost to the
Contractor.
The selected Bidder will ensure staff providing services at Glencliff Home are
informed of and knowledgeable of applicable state and federal privacy and
confidentiality laws and regulations.
The selected Bidder must ensure all employees assigned to perform work
under this agreement possess the necessary knowledge and ability to perform
assigned work and comply with all COVID-19 protocols required by the Glencliff
Home for entry to the premises.
Warranty
2.19.1. Except as otherwise specified, all new parts and labor must be
guaranteed by the selected Bidder against defects resulting from the
use of inferior materials, equipment or workmanship for one (1) year
from the date of acceptance of work by the Department.
2.19.2. If, within any guarantee period, repairs or changes are required in
connection with guaranteed work, which in the opinion of the Contract
Administrator, is rendered necessary as a result of the use of
materials, equipment or workmanship which are inferior, defective, or
not in accordance with the terms of the resulting contract, the selected
Bidder must, promptly upon receipt of notice from the Department, and
at the selected Bidder’s own expense:
2.19.2.1. Place in satisfactory condition in every particular, all such
guaranteed work and correct all defects therein.

2.19.2.2.
Make good all damage to the building or site, or
equipment or contents thereof, which in the opinion of the
Contract Administrator, is the result of the use of
materials, equipment or workmanship, which are inferior,
defective, or not in accordance with the terms of the
resulting contract.
2.19.2.3.
Make good any work or material, or the equipment and
contents of said building or site disturbed in fulfilling any
such guarantee
3. Notices
Exceptions
3.1.1. The Department will require the successful Bidder to execute a contract
using the Form P-37, General Provisions and Standard Exhibits, which
are attached as Appendix A. To the extent that a Bidder believes that
exceptions to Appendix A will be necessary for the Bidder to enter into an
Agreement, the Bidder must note those issues during the RFB Question
Period in Subsection 5.2, Timetable. Bidders may not request exceptions
to the Scope of Services or any other sections of this RFB.
3.1.2. The Department will review requested exceptions and accept, reject or
note that it is open to negotiation of the proposed exception at its sole
discretion.
3.1.3. If the Department accepts a Bidder’s exception, the Department will, at
the conclusion of the RFB Question Period, provide notice to all potential
Bidders of the exceptions that have been accepted and indicate that
exception is available to all potential Bidders by publication of the
Department’s responses on or about the date indicated in Subsection 5.2,
Timetable.
3.1.4. Any exceptions to the standard form contract and exhibits that are not
raised by a Bidder during the RFB Question Period will not be considered.
In no event is a Bidder to submit its own standard contract terms and
conditions as a replacement for the Department’s terms in response to
this solicitation.
RFB Amendment
3.2.1. The Department reserves the right to amend this RFB, as it deems
appropriate, prior to the Bidder submission deadline on its own initiative
or in response to issues raised through Bidder questions. In the event of
an amendment to the RFB, the Department, at its sole discretion, may
extend the Bidder submission deadline. The amended language will be
posted on the Department’s website.
Compliance
3.3.1. Bidders must be in compliance with applicable federal and state laws,
