STATEMENT OF WORK FOR THE OKLAHOMA CITY VA HEALTH CARE SYSTEM Otoscopes/Opthalmoscopes
Background
The Oklahoma City VA Health Care System (OKC VAHCS) requires 13 Otoscope/Ophthalmoscope for Primary Care Service Women s Health Clinic.
Objectives/Scope
To ensure the safety and comfort of patients due to new facility. The exam will provide new equipment for the OKC VAHCS. The vendor will provide the equipment and required accessories for use at the Oklahoma City VA Health Care System. The vendor will inspect all equipment prior to delivery and installation; any concealed damaged components will be replaced at no cost to the VA. Purchase of American Made supplies remains a priority for the VA.
Selection Criteria
Must have Coaxial LED ophthalmoscope
Must have LED otoscope
Must include ear specula dispenser for disposable ear tips which attaches and hangs on wall
Otoscope and ophthalmoscope must be wall mounted with holders for both
Must have automatic on/off sensors
Deliverables or Deliver Schedule
The contractor shall deliver the units standard delivery with F.O.B. destination with 180 days of the date of the award.
The equipment shall be delivered to the following address:

Documentation necessary includes a statement acknowledgement/invoice and receipt of delivery.

Period of Performance
