OHP 0060 9/23 HP-7 Arrest Report (Uniform Traffic Ticket) Five (5) Part Carbonless Form All parts print 2 sided, head to head, with close registration, except Part 2, which prints front only. Red crash consecutive numbering shall start at C541601 and end with C548000. There shall be no missing or duplicate numbers. Collate White CB, Canary CFB with carbonizing tissue tipped at the foot, Blue CFB, Pink CFB, Buff CF, 5/8” perforated stub at top. Perforate 5/16” from the bottom with tissue on Part 2.

UNIT PRICE AWARD: Bidder shall not insert a unit cost more than 3 digits after the decimal point. Digit(s) beyond 3, after the decimal point shall be dropped by DAS and not used in evaluation and any subsequent award. To determine the low lot total price of the ITB, the state will multiply the estimated usage of each item by its corresponding unit price and add the totals together. Failure to bid all items will disqualify your bid.
