Healthy Heart Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) Program in Pathways Community HUBs to improve blood pressure management by supporting community health workers (CHWs), or their equivalents, to implement the Healthy Heart Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) Program. To accomplish this, ODH will contract with a vendor that will provide oversight to Pathways Community HUBs, Care Coordination Agencies (CCA) and CHWs and support program implementation.

Each Offeror must submit a technical proposal and cost proposal as part of its total proposal before the closing date and time, which is 0 5/28/2 026 at 3 PM.
