TO ALL PROSPECTIVE BIDDERS:
The following questions were submitted by a potential bidder. Our answers are listed
below in red:
General Submission & Format
1. Excel Scorecard:
Should pricing be added as a 3rd tab to the scorecard? If not, where would
you like us to breakdown the pricing? Pricing should not be added to the
scorecard. Please include pricing within your proposal.
o If included in Excel, should vendors submit the Excel file with
formulas intact or as values only? Pricing should not be added
to the Excel file.
Are there any restrictions on adding explanatory notes in the yellow cells?
No restrictions.
Can you clarify if references should be added as a tab to the scorecard, or
if that is to be provided as a separate document? References should not
be added to the scorecard.
Can you confirm if on page 25-26, items 3, 5, 6 and 7 should be
incorporated into the Excel document or provided as a separate
document? These items should not be incorporated in the Excel document.
LOSB & Compliance
2. LOSB Goal:
If a vendor cannot meet the 20% LOSB goal, what documentation is
considered sufficient for “good faith effort” on Form B? Your outreach
efforts, ie. emails, phone calls/records, advertisements, etc.
Scope of Work
3. DOT Compliance:
Can you provide a listing of the various job classifications the physicals
would be required for? Firefighters, Deputy Sheriff, Corrections officer,
Corrections Deputy, Roads and Bridges, Support Services, Code Enforcement,
Corrections skilled labor, vector control, pollution, Food & Safety, Correction
dietary, Correction maintenance, Crossing guard, Counselor B, Environmental.
Are you wanting a standard price for all job classifications or you looking for
necessary testing by job classification? Necessary testing by job
classification
Are DOT physicals required for all new hires or only for specific job
classifications? Only for specific job classification
o Should vendors include pricing for DOT exams separately?
Yes
4. Specialty Screenings:
Are there any mandatory screenings beyond standard physicals (e.g., TB,
drug testing, vision)? Yes, TB screening, QFT lab, drug testing, Pulmonary
Function test (PFT), OSHA respirator evaluation and EKG.
Should pricing for these be itemized for each option? Yes, these should be
itemized for each option.
5. Volume & Turnaround:
Is the 24-hour completion requirement for physicals applicable year-round
or only during peak hiring? This is applicable year-round. This involves
Occupational /Employee Health Clinic (OEHC) making the vendor aware of
physicals within 24 hours of notification by the requesting County HR
representative.
Can the County provide historical and/or projected annual volumes of
standard, non-standard, and transportation physicals by department or job
classification, as well as any seasonal peaks in hiring, to allow proposers to
appropriately size staffing and pricing for this contract? Historically, there
have been 300-400 physicals completed per calendar year per outside
vendor. The peak season ranges from Jan.- May and July- Oct.
6. Forms
Does Shelby County currently use standardized examination protocols and
forms across all departments for pre-employment physicals, or are there
department-specific protocols (e.g., Sheriff, Public Works, Transportation)?
If department-specific, will the County provide these protocols and any
required forms with sufficient lead time prior to contract start?” The County
does not provide standardized examination forms. There are department
specific requirements for physical.
Evaluation & Scoring
7. Scoring Weight:
The Excel file shows Department Requirements (60%) and Cost (40%)—
will LOSB compliance affect scoring beyond responsiveness? This cannot
be disclosed.
Contract Terms
8. Renewal Options:
The RFP mentions two optional one-year renewals—will pricing
adjustments be allowed for renewals? Yes
Is there a cap on annual price increases? 5% maximum price increase per
renewal period.
General
Could you please confirm who the current service provider is for this contract?
Currently there is a month-to-month service with Concentra.
Can you provide a breakdown of your current rates by line item? Requires Public
Records Request
What was the total expenditure for these services in fiscal year 2025 YTD?

What has been budgeted for this new contract? Unable to disclose budget
We occasionally utilize 1099 personnel who are trained and managed under our
internal protocols. Can you confirm whether these individuals would be
considered subcontractors under this RFP? Subcontractors submitted to meet
SCG LOSB goal requirements must have a current SCG LOSB certification at
the time of bid.
Are you anticipating the selection of one provider or multiple providers? The
expectation is for one company that can provide more than one provider with
different specialties, such MD, PT, APRN, and OT, if needed.
What are any challenges you are currently facing? There are currently no
challenges.
For the successful Vendor, what implementation timeline and key milestones
does the County anticipate between notice of award and go-live (e.g., contract
execution, protocol review, test transmissions, staff orientation), and will there be
a transition period from any incumbent provider that proposers should plan for in
their staffing and work plan? The successful vendor should be available to fulfill
contract requirements July 1, 2026.
Given the breadth and complexity of the requirements and the recent holiday,
would you consider extending the submission deadline by 1 week to ensure
vendors can provide the most comprehensive and responsive proposals. Yes,
addendum forthcoming.
