Bidders are hereby notified that this Addendum shall be made a part of the documents. The intent is to add to, modify, or
clarify the documents. Should any of these items have an effect on price, such changes shall be included in the price bid.
These items have the same force and effect as if contained in the original.
ITEM 1:
QUESTION/ANSWER
Q1. Can you provide additional specifications/information regarding the type of rebar coupler to be used? Can we
substitute with the "Zap Screwlock SL series"?
A1. The rebar coupler shall meet the requirements of the FDOT Standard Specifications Section 415 (415-5.4.1). Products
that are on the FDOT Approved Products List are acceptable to be used.
Q2. Can you provide additional information on where the below bid items apply to? We did not see any reference
of the bid items on the plan.
A2. Pay Item 460-1-13 STRUCTURAL STEEL REHAB (BOLTS, NUTS, PLATES) is no longer applicable and should be
removed. Pay Item 460-999 STEEL ANCHORS refers to the S-9 Williams Undercut Anchors shown at the interface between
the existing bent cap and proposed cap enlargement, shown in plan sheets B1-12 thru B1-15, and B2-11 thru B2-15. There
are 110 of them being used on Bridge 014044 and 110 being used on Bridge 014045. The value shown, 670 is outdated,
as this originally included dowels at the exist. prestressed beam and sidewalk/curb connections, which are now being
included in the superstructure rebar quantities (Bar 5S1). 110 should be the totals used for both bridges for this pay item.
CORRECTIONS ON BID FORM:
On Page 49, Bridge #014044 Chamberlain Boulevard over Jupiter Waterway:
o Bid Item #460-1-13 – Structural Steel Rehab (Bolts, Nuts, Plates) - please strike through this line.
o Bid Item #460-999 – Steel Anchors - please correct quantity from 670 to 110.
On Page 51, Bridge #014045 Chamberlain Boulevard over Apollo Waterway:
o Bid Item #460-1-13 – Structural Steel Rehab (Bolts, Nuts, Plates) - please strike through this line.
o Bid Item #460-999 – Steel Anchors – please correct quantity from 710 to 110.
Q3. Will the class 5 coating be applied only to the graphic area in the railing? See Sheet B1-24, Note #4 (BR 014044).
Or will the coating be applied to the entire railing. There is no bid item for Class 5 Coating.
A3. The Class 5 coating is only intended to be applied to the graphic area on the railing. Per Note 5 on Sheet B1-24, the
cost of labor and materials required to produce/install the graphics onto the railings shall be included in Pay Item 521-5-99,
CONCRETE TRAFFIC RAILING-BRIDGE 42”.
Q4. On Bridge 014044 - The existing Buried Fiber Optic line will be interfering with the construction of the new
sidewalk on the bridge? Will the private utility provider be responsible for the relocation?
A4. Yes, the private utility owners will be responsible for relocating their line prior to construction.
NOTE TO BIDDERS: Please make sure to strike through Bid Items #460-1-13 and correct the quantities on Bid
Items #460-999 to 110 on Page 49 and Page 51, before submitting the bid.
This addendum is binding and is to be considered as if contained within the original bid documents of Bid No. 20260277.
Bidders are required to acknowledge receipt of this addendum on their bid forms.
