Notice of Intent to Award – Multiple Blanket Purchase Agreements

In accordance with FAR 13.303-2(c)(1), the Navajo Area Indian Health Service, Shiprock Service Unit located in Shiprock, New Mexico, intends to place multiple-award Blanket Purchase Agreements (BPA) with Heritage DME and Oaktree Products Inc. for miscellaneous types of clinal rehabilitation supplies foam & cotton ear dams, ear lights, probe tubes, insert tips, tubing single use ear tips, battery testers and extractors, hearing instrument cleaners, pocket talkers, electrodes, acupuncture needles, pelvic health rehabilitation devices and supplies, upper and lower extremity braces, adaptive equipment examination tools, on an as-needed basis.

The anticipated period of performance is a base period of one year from the date of award with (4) one year option periods.

Base Year: Date of Award to February 28, 2026

1st Option Year: March 1, 2026 to February 28, 2027

2nd Option Year: March 1, 2027 to February 29, 2028

3rd Option Year: March 1, 2028 to February 28, 2029

4th Option Year: March 1, 2029 to February 28, 2030

The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339112 – Surgical and Medical Instrument Manufacturing with a small business size standard of 1,000 employees.

No further awards will be made unless the need for Shiprock Service Unit located at Shiprock, New Mexico. The prospective, multiple-award BPAs as identified in this notice are sufficient at this time. In accordance with FAR 13.303-2(c)(1), establishment of multiple BPAs constitute maximum practicable competition.

Any other sources that believe they are capable of fulfilling the government requirements as described are encouraged to submit their capability statement, price lists, and catalogs to the Point of Contact listed below via email only and no later than March 10, 2025 at 10:00 an MDT. In the event additional BPAs are deemed to be in the best interest of the government, capable vendors may be contacted.
