RELEASE OF CLAIMS FOR PUNITIVE DAMAGES

This release concerns The Board of Trustees of The University of Alabama, a public

educational and constitutional instrumentality of the State of Alabama, incorporated by statute, for

and on behalf of The University of Alabama in Tuscaloosa (herein, the “University”), and NAME OF

DEALERSHIP, doing business in STATE located at DEALERSHIP ADDRESS, CITY, STATE,

ZIP CODE (herein, “Dealer”).

In consideration of Dealer’s waiver of its arbitration requirement and incident to the

University’s purchase of a certain motor vehicle QUANTITY YEAR MAKE MODEL OF VEHCILE

BEING PURCHASED from Dealer, the University on behalf of itself, its successors, and assigns,

hereby fully releases and discharges Dealer, its successors, and assigns, from recovery by the

University of any punitive damages incident to any rights, claims, or actions which the University and

its successors and assigns may have now or in the future against Dealer incident to and growing out

E of the University’s purchase of the motor vehicle from Dealer, under the terms of sale of the motor

vehicle earlier described hereinabove in this paragraph, which sale documents are attached hereto

SAMPL and incorporated by reference making them a part of this release.
