Project Description: Issuing Procurement Unit: State of Utah Division of Purchasing
Conducting Procurement Unit: Utah Department of Health and Human Services (DHHS)
Professional Services Requested: Plan, coordinate, and host peer-run conferences for peer support specialists and family peer support specialists
Length of Contract: five (5) years
The purpose of this RFSQ is to obtain statements of qualifications from companies. The intent is to award a contract under Utah Procurement Code 63G-6a Part 15 for the purchase of Design Professional Service Providers and Consultants. The vendor with the highest- ranking will be selected to negotiate a contract at compensation determined to be fair and reasonable. If fair and reasonable compensation, contract requirements, and contract documents cannot be agreed upon with the highest ranked vendor, the state may proceed with negotiations with the next highest ranked vendor. Do NOT include pricing when you submit your qualifications.
Additional information about this project: The budget for this project is $150,000.00 for the entire 5-year term ($30,000 per year). This will be funded, in whole or in part, by DHHS' Mental Health Block Grant, which has federal restrictions. With DHHS permission, the awarded vendor may also do a limited aount of fundraising to cover conference expenses that cannot be expensed under DHHS' Mental Health Block Grant. Over the course of the 5-year contract term, DHHS reserves the right to change funding sources, which may include, but is not limited to, identifying new funding sources to expand the program. Adding funds to the resulting contract above the inital $150,000.00 will require approval from the State of Utah's Chief Procurement Officer (via the State of Utah Division of Purchasing) and a contract amendment.
