| Description: | The City of Crystal Lake is requesting proposals from professional vendors with experience and expertise to complete mosquito management services. Services will include the following: • General Service • Surveillance and Monitoring • Larval Control • Adult Control Vendors must apply all insecticides in strict conformance with procedures and application rates approved by the Illinois Department of Agriculture and the United States Environmental Protection Agency (EPA). All aerial operations shall conform to Federal Aviation Authority rules and regulations, along with other local and state regulations. If required by federal or state law, the vendor must be permitted by the EPA/IEPA under the Nation Pollutant Discharge Elimination System (NPDES) for pesticide application point source discharges at least 30 days prior to any application. Vendor must prepare and submit the annual NPDES permit report, if required, on behalf of the City in accordance with Pesticide Discharge Management Plan. Vendor must employ adequate number (minimum of 15) of Illinois Department of Agriculture licensed applicator and operators and have vehicles located and registered in Illinois, equipped to handle large scale mosquito control operations. As part of the submittal, vendor shall provide lists from 2024 and 2025 that include names, pesticide license numbers and expiration dates of all Illinois Department of Agriculture licensed applications. |
