Mobile Lithotripsy

SUNY UPSTATE MEDICAL UNIVERSITY (“UPSTATE”) intends to make an award under our discretionary authority, pursuant to NYS Education Law §355(5)(a), which is up to $125,000, for a vendor to provide Mobile Lithotripsy services. A sealed bid will not be held. If you are interested in providing a proposal for the above service, you must contact the appropriately designated Campus contact in this advertisement within fifteen (15) business days of this Advertised Notice.

Requirements:

• Must be able to do business in New York State.
• The vendor must have over five (5) continuous years’ experience providing mobile lithotripsy services.
• Vendor shall use the same or similar lithotripsy device as a LithoGold Lithotripter, Table and C-Arm Model: LG 380 Serial: 30
• Vendor must have a back-up plan in case of device failure or emergency coverage, including a back-up lithotripsy device stored locally if needed.
• Vendor must provide technologists to provide the services, who have ARRT certification, CRLS certification, or state licensure.
• Vendor must have policies, procedures and/or quality control programs in place to ensure regulatory compliance and accuracy with mobile lithotripsy services.
• Vendor must have privacy policies with respect to the customer and consumer information for mobile lithotripsy purposes.
• Vendor must be able to provide Customer Service Support 24 hours, 7 days a week, 365 days a year.

This procurement is subject to NYS Finance Law (“SFL”) §§139j&k, also known as the Lobbying Law, therefore, vendors are cautioned that the only permissible contact for this procurement is the individual(s) listed below. Contacting any other individuals at Upstate in connection with this advertisement may constitute a violation of SFL. Failure to comply with this requirement can range from no contract award to debarment from sales to any NYS agency. Vendors responding to this advertisement shall be required to complete and submit applicable documents related to Vendor Responsibility, NYS SFL §§139 j&k and NYS Tax Law Section 5-a.

Vendors responding to this advertisement will be required to submit valid and current NYS Workers’ Compensation and Disability forms.
