excluding travel expenses, which will be reimbursed to Contractor in accordance with the rates currently established

Iowa State University of Science and Technology (ISU) is soliciting bids for Medical Waste/Biohazardous collection
and removal from 3 ISU locations – Environmental Health & Safety, Vet Med and the Student Health Center.

2 EVALUATION CRITERIA
Iowa State University reserve the right to select the Contractor(s) that are in the best interest of each University.
The evaluation criteria utilized for this RFP may be based upon, but not limited to, the following criteria which are
listed in no particular order:
Bidder's ability to meet the scope of work listed in Attribute #7
Overall cost to ISU of all proposed services
Qualifications and experience
Exceptions taken to the RFQ terms and conditions

3 TERM
This contract will begin on approximately February 1st, 2024 and be good for a 3 year initial term with two additional
1-year optional renewals upon mutual agreement.

4 OVERVIEW
This RFP and responding Proposals shall establish a price Contract for the pick-up and disposal of regulated
medical waste (RMW) which can include biohazardous waste, infectious medical waste, and waste contaminated
with blood, body fluids, or other potentially infectious materials. This waste may include sharps, pathologic waste,
“red bag waste,” bacteriology media plates, blood tubes, pipette tips, pharmaceutical waste, and other
contaminated items.
The Company agrees to provide the University the service of removal and hauling of the described medical waste
onto transportation vehicles, the transportation of the described medical waste to an authorized disposal facility,
and the disposal of the described medical waste, in a manner permitted by law.

5 PICK-UP LOCATIONS
Contractor will be responsible for picking up waste at the following locations:
Environmental Health & Safety
Theilen Student Health Center
ISU College of Veterinary Medicine
Vet Teaching Hospital including Large Animal Hospital, Small Animal Hospital
Clinical Pathology

6 PICK-UP SCHEDULE
Company shall coordinate the medical waste/biohazard pick-up with the University Representatives for the services-
Jessica Shannon, for the Environmental Health & Services and Student Health Center locations.
The Environmental Health & Safety requires a monthly pick-up.
The Student Health Center requires a weekly pick-up.
The College of Vet Med requires a weekly pick-up.
University representatives may request an additional pickup with 48 hours advance notice.

7 SPECIFICATIONS
Company agrees to pick-up and dispose of regulated medical waste (RMW) which can include biohazardous waste,
infectious medical waste, and waste contaminated with blood, body fluids, or other potentially infectious materials.
This waste may include sharps, pathologic waste, “red bag waste,” bacteriology media plates, blood tubes, pipette
tips, pharmaceutical waste, and other contaminated items.
Company agrees to furnish all packing and shipping containers and maintain an adequate number of packing and
shipping containers on site for use to transport the medical waste.
The weight of packing material and shipping containers shall not be included when calculating disposal cost.
The calculations of weight of medical waste shall not include the packing and shipping container weight.
Company shall provide a list of container sizes that are available.
Company shall provide a list of medical waste acceptable for pick-up and disposal.
Company shall provide a list of pricing of disposal of medical waste per type, per pound.
Company shall provide a list of any other applicable fees for removal and hauling of medical waste from University
locations.
All medical waste must be transported and disposed of in conjunction with current and existing EPA,
OSHA, DOT, State of Iowa, local and federal regulations and requirements.

8 Value Added Services
