MEDICAL TESTING SERVICES

Project Description: SCOPE: The Niagara Frontier Transportation Authority (NFTA) hereby seeks qualified contractors to submit proposals that meet the minimum acceptable criteria set forth herein to provide Medical Testing Services for NFTA and Niagara Frontier Transit Metro Systems, Inc.(NFTM) employees as required by OSHA CFR Part 1910 (www.osha.gov). Medical services will be performed on and offsite at set scheduled times and include Hepatitis B Immunizations, Respirator Questionnaires, Pulmonary Tests, Fit Tests and Electrocardiograms.

The prospective contractor is required to become familiar with the work covered by this contract prior to submitting a proposal.

For successful questionnaire and /or Bid/Table template uploads, please ensure you are using Microsoft Office/Excel/Word 2007 or newer.
