REQUEST FOR PROPOSALS (RFP)
Part-Time Physical Therapist Services
RFP Issue Date: Monday, May 18, 2026
Proposal Due Date: Friday, June 5, 2026
Anticipated Start Date: Beginning with the dates identified on the attached 2026-27 Academic
Calendar or earlier, as requested, for needs during the summer of 2026
1. PURPOSE
The Middletown Area School District ("District") is seeking proposals from qualified individuals
or businesses to provide part-time physical therapy services for eligible students in accordance
with IEPs, Section 504 Plans, and applicable regulations.
2. BACKGROUND
The District serves students in grades K-12 and is committed to providing high-quality special
education services. Beginning with the 2026-2027 school year, all district schools are scheduled
to be located on one campus.
3. SCOPE OF SERVICES
- Provide evaluations and re-evaluations, including early intervention transition that could occur
during the summer
- Develop and implement therapy plans aligned with IEP goals
- Deliver direct and consultative services
- Obtain and/or maintain compliant documentation, including, but not limited to, progress
monitoring records, School-Based medical access billing, physician authorizations.
- Attend IEP meetings, in-person or remote as authorized by Director of Special Education
- Collaborate with staff, families and stakeholders
- Recommend equipment/modifications and obtain equipment at the lowest possible price in
coordination with the Director of Special Education or designee

Level of Effort: Part-time, approximately 25-30 hours per week during the academic year. Any
hours needed beyond 25-30 hours per week must be authorized by the Director of Special
Education in advance.
4. QUALIFICATIONS
- PA Licensed Physical Therapist
- School-based experience in a low incidence (multiple disabilities, life skills, autism) classroom
preferred
- Knowledge of IDEA and Section 504 of the Rehabilitation act of 1973
- Completion of child abuse training
- Required clearances (Act 34, 151, 114, 168)
- Proof of liability insurance
5. CONTRACT TERM -
The initial contract term will be for one (1), three (3) or five (5) years beginning July 1, 2026.
Contract can be cancelled with a 30-day written notice by the District prior to the expiration of
the contract if the District is not satisfied with the services being provided or due to negligence
of the provider.
6. PROPOSAL REQUIREMENTS
Cover Letter, Qualifications of Vendor and Proposed Therapist (including licenses and
certifications), Cost Proposal, References (school districts preferred), Vendor Response to RFP
Questions, Form W-9, Completed Agreement- with the exception of the term to be decided by
the District

8. SUBMISSION INSTRUCTIONS
Proposals shall be submitted in a sealed envelope that displays "RFP Response for Physical
Therapist Services" by Friday, June 5, 2026 at 10:00 AM to Middletown Area School District, 55
West Water Street, Middletown, PA 17057.
9. TERMS AND CONDITIONS
District reserves rights to reject proposals and waive irregularities. District may award contracts
to multiple vendors in order to meet the service demand of the District.

Agreement with Vendor for Part-Time Physical Therapy Services
This Agreement ("Agreement") is entered into this ___ day of __________, 2026, by and
between Middletown Area School District, located at 55 West Water Street, Middletown,
Pennsylvania 17057 ("District"), and __________________________________________
("Contractor").
1. PURPOSE
The District desires to obtain part-time physical therapy services for eligible students in
accordance with Individualized Education Programs ("IEPs"), Section 504 Plans, and
applicable federal and state laws and regulations. Contractor agrees to provide such
services under the terms and conditions set forth herein and pursuant to the District's
Request for Proposals ("RFP") for Part-Time Physical Therapist Services dated May 18,
2026.
2. TERM (to be chosen by the District. Vendor should leave blank.)
(To be chosen by the District) The Agreement shall commence on July 1, 2026 and
continue for a term of:
One (1) Year
Three (3) Years
Five (5) Years
unless terminated earlier in accordance with this Agreement.
The anticipated service start date shall begin with the 2026-2027 academic calendar or
earlier, as requested for summer 2026 needs.
3. SCOPE OF SERVICES
Contractor shall provide qualified physical therapy services including, but not limited to:
a. Provide evaluations and re-evaluations, including early intervention transitions that
occur during the summer
b. Develop and implement of therapy plans aligned with IEP goals;
c. Deliver direct and consultative therapy services;
d. Obtain and/or maintain compliant documentation, including, but not limited to, progress
monitoring records, School-Based medical access billing, physician authorizations
e. Participation in IEP meetings in person or remotely, as authorized by the Director of
Special Education;
f. Collaboration with District staff, families, and stakeholders;
g. Make recommendations regarding adaptive equipment and modifications; and
h. Coordination with the Director of Special Education or designee to obtain equipment
at the lowest possible cost.
Services may be performed at:
* Middletown Area High School
* Middletown Area Middle School
* Reid Elementary School
* Middletown Area Primary School
or other District-approved locations.
The anticipated level of effort is approximately twenty-five (25) to thirty (30) hours per
week during the academic year. Any hours exceeding this amount must receive prior
approval from the Director of Special Education.
4. CONTRACTOR QUALIFICATIONS
Contractor certifies that all personnel providing services under this Agreement shall:
a. Hold a current Pennsylvania Physical Therapist license;
b. Maintain all legally required certifications and clearances, including but not limited
to Act 34, Act 151, Act 114, and Act 168 clearances
c. Possess school-based experience in low incidence (multiple disabilities, life skills,
autism) classrooms preferred
d. Proof of liability insurance
e. Knowledge of IDEA and Section 540 of the Rehabilitation Act of 1973
f. Completion of Child Abuse Training
Contractor shall immediately notify the District if any required license or clearance lapses,
is suspended, or revoked.
5. COMPENSATION
The District shall compensate Contractor in accordance with the pricing submitted in
Contractor's proposal and accepted by the District.
Payment terms shall be as follows:
Contractor shall submit invoices bi-weekly or monthly to the District. The District shall
remit payment in accordance with normal District payment procedures, or Net 20.
6. INDEPENDENT CONTRACTOR STATUS
Contractor is an independent contractor and shall not be considered an employee of the
District. Contractor shall be solely responsible for all compensation, taxes, insurance,
benefits, and obligations relating to its employees or subcontractors.
7. INSURANCE
Contractor shall maintain throughout the term of this Agreement:
* Professional Liability Insurance;
* General Liability Insurance; and
* Workers' Compensation Insurance, if applicable.
Proof of insurance shall be provided to the District prior to commencement of services
and upon request thereafter.
8. CONFIDENTIALITY
Contractor agrees to maintain the confidentiality of all student records and personally
identifiable information in accordance with the Family Educational Rights and Privacy Act
("FERPA"), IDEA, HIPAA where applicable, and all other applicable laws.
Contractor shall not disclose confidential information except as permitted by law or
authorized by the District.
9. COMPLIANCE WITH LAWS
Contractor shall comply with all applicable federal, state, and local laws, regulations,
board policies, and professional standards, including but not limited to IDEA, Section 504
of the Rehabilitation Act of 1973, and Pennsylvania Department of Education
requirements.
10. NON-DISCRIMINATION
Contractor agrees not to discriminate against any employee, applicant, student, or
participant on the basis of race, color, national origin, religion, sex, age, disability, veteran
status, or any other protected classification under applicable law.
11. TERMINATION
The District may terminate this Agreement with thirty (30) days written notice if:
a. The District is dissatisfied with the services provided;
b. Contractor is negligent in the performance of services;
c. Contractor breaches any material term of this Agreement; or
d. Funding for services becomes unavailable.
The District may terminate this Agreement immediately for violations involving student
safety, loss of required licensure or clearances, or unlawful conduct.
Contractor may terminate this Agreement with thirty (30) days written notice to the
District.
12. INDEMNIFICATION
Contractor agrees to indemnify, defend, and hold harmless the District, its board
members, employees, and agents from and against any claims, damages, losses, liabilities,
and expenses arising out of Contractor's negligent acts, errors, omissions, or breach of
this Agreement.
13. ASSIGNMENT
Contractor may not assign or transfer this Agreement without prior written consent of
the District.
14. ENTIRE AGREEMENT
This Agreement, together with the RFP and Contractor's submitted proposal, constitutes
the entire agreement between the parties and supersedes all prior negotiations or
understandings.
15. GOVERNING LAW
This Agreement shall be governed by and construed in accordance with the laws of the
Commonwealth of Pennsylvania.
16. SEVERABILITY
If any provision of this Agreement is found invalid or unenforceable, the remaining
provisions shall remain in full force and effect.
