Lithotripsy Services

SUNY Downstate Health Sciences University, (SUNY-DHSU) hereby requests proposals from qualified firms to provide Unilateral and Bi-lateral lithotripsy services for the Perioperative Department at University Hospital, located in Brooklyn, New York 11203.

• Equipment: Contractor to provide its own lithotripsy equipment.
• Personnel / Services: Contractor to provide trained and certified lithotripsy technologists and other personnel to operate the equipment and assist with procedures.
• Submitted Proposals must include per-procedure fee schedule.
• Invoices for Services rendered shall be structured on a per-procedure fee basis.
• Regulatory compliance: All services rendered by Contractor shall be consistent with applicable regulatory compliance.
