Licensed Physician Services for Physical Examinations

Vendor shall provide the Agency with licensed physician services for physical examinations. Physical examinations must be performed by a licensed physician who possesses all requirements listed in the specifications. The Agency shall pay a flat fee for each physical examination, as shown on the Pricing Page, for all Contract Services performed and accepted under this Contract. The cost of all lab work, testing and procedures contemplated herein is included in the unit cost of Contract Service #1 - Physical Examinations. Vendor is responsible for all costs associated with the performance of this Contract. Such costs may be included in the flat fee listed on Vendor's bid, but will not be paid by the Agency separately.
