1. Scope of Work
This requirement needs to provide lead aprons, thyroid shields, protective eyewear, and apron racks, with many items requiring custom embroidery and specific staff name tags.
2. Place of Performance
The product is requested to be delivered to Surgery Service, building 100, MOR.
10,000 Bay Pines Blvd, Bay Pines, Florida 33744, via the Warehouse Building 36 for Equipment.
3. Period of Performance
The product is requested to be delivered no later than December 2nd.
4. Requirement Overview
The Contractor shall provide the full requested quantity of NAME BRAND OR EQUAL products listed below.
Salient Characteristics
General Requirements for Lead protection eyewear:

0.75 mm Pb equivalence or greater
Must have anti-reflective and anti-fog included
Outside engraving with staff name or Operating Room
Must provide Inservice for sizing to ensure correct fit
Must be certified to ANSI Z87.1 and ASTM radiation protective eyewear standards
Must include engraved or permanent labeling indicating Pb equivalency, manufacture and compliance with safety standards

General Requirements for vests and aprons:
FDA-approved for radiation protective use.
Compliant with OSHA, NRC, ANSI, and Joint Commission standards.
0.5 mm Pb equivalence at 100 kVp or higher.
Latex-free, wipeable, suitable for hospital-grade disinfectants.
Must be new, unused, and one-year warranty.
Protective material must come in the following choices: Lead - 0.5mm (Heavy), Lead - 0.35mm (Heavy), Lead - 0.25mm (Heavy)
Must include embroidery option with names, locations and color pattern choices
Vendor must provide sizing/fitting consultation
Must have Velcro name tape options
Must be available in S, M, L, XL and custom to accommodate staff
Must be compatible with standard apron racks.
Must come with outer fabric choices, inner fabric choices, binding color choice, and thread color choices

Kilt
Must be available in .50mm, .35mm, or .25mm front protection
Must be customizable with various colors and patterns and embroidery options
Must come with 0.50mm protection using .25mm on each flap and the back panel of the kilt must have 0.25mm thickness.
Sizes must range from male extra small to extra-large and female extra small to extra large

Thyroid Shield
Thyroid Bib Dimensions must be a minimum of 5.89 W x 4.5 D
Must come in Small (23 ) Necks 11 -15 , Medium (27 ) Necks 15 -19 , Large (31 ) Necks 19 -23
Closure must come in magnet, hook and loop or buckle
Must have option to tether ring, tether to tether or none
BAT - Breast, Axilla and Thyroid Radiation Protection
Must be able to be worn over or under a current radiation protection apron
Must have a minimum of 0.35 mm protection in the main body, sleeves, and breastplate
Must come with adjustable hook and loop closure for a custom fit
Thyroid Shield Closure must come with magnet, hook and loop or buckle options
