Kayenta Health Center: Non-Personal Service Contract - Licensed Optometrist Services

The Indian Health Service, Kayenta Health Center, located in/at Highway 160, Mile Post 394.3 has an upcoming requirement for two (2) non-personal service contracts for two (2) Optometrists.

The Period of Performance is a base period plus three (3) option periods (six (6) months per period):

Base Period: 09/01/2025 through 02/28/2026

1st Option Period: 03/01/2026 through 08/31/2026

2nd Option Period: 09/01/2026 through 02/28/2027

3rd Option Period: 03/01/2027 through 08/31/2027

The North American Industry Classification System or NAICS code is 621320 – Offices of Optometrists with a small business size standard of $9.0 million.

The Request for Quotations No. 75H71025Q00033 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Acquisition Procedures will be issued on or by August 5, 2025.

Evaluation factors may include:

• Qualifications of the Vendor, technical capability, and approach in order to meet the government’s requirement as defined in the Performance Work Statement (PWS).


• Experience (Licensed Optometrist Key Personnel)
  
  
  
  • Key Personnel’s licenses/certifications – BLS, ACLS, PALS, Optometry License, Diagnostic and Therapeutic License, and DEA license.

Quotes will be due on or by August 20, 2025. The acquisition is a 100% Small Business set aside.
