Description
Contractor shall furnish all labor, supervision, travel expenses, materials, parts, tools and equipment necessary for the preventative maintenance and emergency repair on the Guldmann ceiling mounted patient lifts at the following facilities:

Clement J. Zablocki VA Medical Center, 5000 National Avenue, Milwaukee, WI 53295
Milo C. Huempfner CBOC, 2821 University Avenue, Green Bay, WI 54311
Union Grove CBOC, 21425 Spring Street, Union Grove, WI 53182
Oconomowoc CBOC, 1289 Olympia Fields Drive, Oconomowoc, WI 53066

Contract base period shall be 10/01/2025 through 09/30/2026 with renewal options for four (4) additional years.
