QUOTE INSTRUCTIONS
November 2025
DESCRIPTION OF WORK
The contractor shall provide all labor, materials, equipment, supervision and supplies as required
for a complete installation of carpet tiles, cove base and necessary materials in several areas at
Science Hill High School as per these documents.
JOBSITE VISIT REQUIRED
Vendor shall visit the jobsite to familiarize themselves with the work conditions and materials
needed and not to be based on the estimated square footage contained in this quote.
LICENSES, FEES, PERMITS
The Contractor is responsible for furnishing the proper licenses, fees, and permits required by law
to do business in the City of Johnson City in completion of the requirements stated herein. All
work shall be done in accordance with the latest building codes, state and federal laws relative to
this project.
INSURANCE
The attached Insurance Checklist (which includes a section for the Insurance agent to fill-out) and
General Contract Form shall be completed and returned with the quote package. Successful vendor
shall provide certificate of insurance, as specified, prior to contract release by Purchasing.
DRUG FREE WORKPLACE
All Contractors must execute the enclosed Drug Free Workplace Affidavit to verify compliance
with TCA 50-9-113 and return same with quote response. Failure to comply with this
requirement will declare that quote non-responsive.
SECTION I – PURPOSE OF THE DRUG & ALCOHOL TESTING PROGRAM
The City of Johnson City recognizes its responsibility to provide safe and efficient operations for
our employees, our citizens and the general public.
Our commitment to provide safe and
efficient operations is shown by the implementation of programs and procedures which ensure
compliance with appropriate safety measures, as well as the letter and intent of all applicable laws
and regulations. There is sufficient evidence to conclude that the use of illegal drug/alcohol;
drug/alcohol dependence
and drug/alcohol abuse seriously impairs an employee’s performance and general physical and
mental health. The illegal possession and use of drugs, alcohol and/or narcotics by employees of
the City is a crime in this jurisdiction and is clearly unacceptable. Therefore, the City of Johnson
City has adopted this written policy to ensure an employee’s fitness for duty as a condition of
employment; to ensure the drug tests and alcohol tests are conducted on safety-sensitive
positions in the categories of: pre-employment, randomtesting, suspicion testing, and return-to-
duty testing.
To comply with TCA Title 50 Chapter 9 Part 1, all bidders and/or those proposing to do service
with the City must have a testing program of the same or better than the requirements of the City of
Johnson City.
STATE CONTRACTORS LICENSE (IF QUOTE IS $25,000+)
Contractor must be a licensed contractor in the State of Tennessee, as required by the Contractor's
Licensing Act of 1994, State of Tennessee. Include a copy of Vendor’s State Contractor’s
License with quote package.

THE GENERAL TERMS & CONDITIONS
This quote also includes the City’s “Requirements for Bids, Requests for Proposals, and Contracts,
etc.” and “General Terms and Conditions” attached hereto and set forth herein as if verbatim.
CRIMINAL HISTORY AFFIDAVIT
All bidders must execute the attached Criminal History Affidavit. Contractor shall comply with
Tennessee Code Annotated Section 49-5-413, which requires all contractors to conduct a criminal
history records check conducted by the Tennessee Bureau of Investigation and the Federal Bureau
of Investigation on each employee prior to permitting the employee to have contact with students or
enter school grounds when students are present.
RIGHTS AND OPTIONS OF THE CITY OF JOHNSON CITY
The City reserves the following rights and options:
• Determine those vendors who are most qualified.
• Reject any or all submittals, for any reason, at its sole discretion.
• Supplement, amend, or otherwise modify this solicitation.
• Cancel this solicitation with or without submission of another solicitation.
• Reserve the right to request additional information as part of this selection process.
• Issue additional solicitations for information and proposals, and conduct
investigations with respect to the qualifications of each respondent.
SPECIFICATIONS
1. General Requirements
1.1 Scope of Work
The Contractor shall furnish all labor, materials, tools, equipment, and supervision
necessary to remove exis�ng carpet, install new Kine�x carpet �les, and provide
associated services as detailed herein for Johnson City Schools.
1.2 Work Schedule
The Contractor shall complete the work within 30 calendar days from the No�ce to
Proceed. Work shall be performed during normal school hours unless otherwise
approved by Johnson City Schools.
1.3 Site Condi�ons
The Contractor shall examine the work site prior to submi�ng a bid and shall be
responsible for verifying all dimensions and condi�ons affec�ng the work.

2.3 Adhesives and Installa�on Materials\
• All adhesives and installa�on materials shall be compa�ble with Kinetex carpet �les
and approved by the manufacturer.
• Adhesives shall be low-VOC and meet all applicable environmental standards.
3 Workmanship
3.1 Installa�on
• Carpet �les shall be installed in accordance with the manufacturer's instruc�ons and
industry best prac�ces.
• Seams shall be �ght and inconspicuous.
• Tiles shall be installed with consistent patern alignment and direc�on.
• Toe boards shall be installed with �ght joints and securely fastened.
3.2 Furniture Handling
• The Contractor shall move all furniture and equipment as necessary to perform the
work.
• Furniture shall be protected from damage during the work.
• Upon comple�on, furniture shall be returned to its original posi�on.
3.3 Site Cleanup
• The Contractor shall maintain a clean work area and remove all debris and waste
materials daily.
• Upon comple�on, the work area shall be le� in a clean and orderly condi�on.
4 Warranty
4.1 Materials Warranty
• The Contractor shall provide a warranty for all materials supplied, covering defects in
materials and workmanship for a period of 10 years from the date of acceptance by
Johnson City Schools.
4.2 Workmanship Warranty
• The Contractor shall provide a warranty for workmanship, covering defects in
installa�on and performance for a period of 10 years from the date of acceptance by
Johnson City Schools.
5 Submitals
5.1 Product Data
• The Contractor shall submit product data for all materials proposed for use, including
manufacturer's specifica�ons, warranty informa�on, and installa�on instruc�ons.
5.2 Samples
• The Contractor shall provide samples of carpet �les and toe boards for approval by
Johnson City Schools prior to ordering materials.
5.3 Insurance and Licenses
• The Contractor shall provide proof of insurance and all necessary licenses as required
by Johnson City Schools.
