Project Description: The Suffolk County Office of Central Procurement, on behalf of the Division of Human Resources, invites Proposals from qualified entities to provide consulting and reporting services to convert data received from the County to prepare, file with the Internal Revenue Service (“IRS”) and distribute forms 1095-C and 1094-C in compliance with the Patient Protection and Affordable Care Act (“PPACA”).
