The Center for Laboratory Sciences (CLS) is seeking to engage a contractor to provide physicians and Licensed Health Care Professionals (LHCPs) with appropriate experience and training in occupational health/medicine to provide the following examinations, vaccinations, medical services, and laboratory tests listed in Exhibit A, Scope of Work. The Center for Laboratory Sciences intends to make a single contract award to the most responsive and responsible firm offering the lowest cost.

Bids must address all of the services described in the Exhibit A entitled, Scope of Work. Contractor agrees to provide the following, but not limited to:
Comprehensive occupational health services for CLS employees, including physical exams, vaccinations, and lab tests. The contractor must have experienced physicians and licensed health care professionals, ensure timely results within 30 days for standard services and 10 working days for urgent requests, and provide clear evidence of service timelines.
