Bid Synopsis: The Wisconsin Department of Health Services (DHS) is seeking proposals for an agency to evaluate, authorize, and oversee the IRIS SDPC services that are offered to eligible IRIS participants enrolled via the 1915(j) State Plan Amendment. DHS intends to use the results of this process to award one Contract. IRIS is Wisconsin’s self-directed long-term care program for frail elders and adults with physical or developmental disabilities to get the services they need to remain in their homes whenever possible and remain independent. IRIS is available to Wisconsin residents determined financially eligible for Medicaid and functionally in need of nursing home or Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) levels of care.
