IFB #25-30304-000 Optometry Services

Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, certifications, permits, insurance and personnel to provide DSH with optometry services on an as-needed basis. Services shall be provided to assist the medical staff at State Hospitals in providing the highest quality of medical care reasonable and consistent with available resources and to assure that appropriate clinical evaluations and documentation meet the requirements and standards set forth by Title 22 of the California Code of Regulations regarding community care licensing, the Joint Commission, and the Department of Justice.

Services shall include but are not limited to: eye examinations; screenings, follow-ups, eye refractions, errors, pressures, and cataracts; prescription of corrective lenses, which shall be provided through the state supplied source; optical dispensing of custom prescription glasses to include fabrications, fittings, and maintenance and repairs for broken or damaged glasses. Screening, treatment and management of glaucoma, management of diabetes and diabetic retinopathy not requiring surgical or medical treatment.
