IFB On-call Pickup, Removal, Cleanup and Disposal of Hazardous Waste in the Counties of Mendocino, Lake, Humboldt, and Del Norte

Contractor shall furnish all labor, equipment, supplies, and materials required to test, identify, and
reduce contamination; neutralize waste; and remove, transport, and legally dispose of waste
substances and other waste materials to Caltrans.
Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid
and current:
Class A General Engineering license issued by the California Contractors State License
Board (CSLB)
Hazardous Substance Removal (HAZ)¿Certification issued by the California Contractors
State License Board (CSLB)
Hazardous Material Certificate of Registration issued by US Department of Transportation
Pipeline and Hazardous Material Safety Administration (Title 49, Code of Federal Regulations, Part 107, Subpart G)
Hazardous Materials Transportation License issued by California Highway Patrol (CHP)
Waste Tire Hauler Registration Decal (for each vehicle with effective and expiration
dates) issued by Cal Recycle (Department of Resources Recycling and Recovery )
Hazardous Waste Transporter Registration issued by the Department of Toxic Substances
Control, California Environmental Protection Agency (CalEPA). (Chapter 6.5, Division 20
Health and Safety Code (H&SC) and CCR, Title 22, Division 4.5
Transporter EPA ID (Section 3010 of the Resource Conservation and Recovery Act)
Bidder shall be registered at the time of bid and throughout duration of contract with the
Department of Industrial Relation (DIR).
Bidder shall possess at the time of bid submittal and for the duration of the Agreement, a valid and
current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV).
Caltrans will verify bidder¿s Motor Carrier Permit. Bidder shall provide a copy with bid submittal.
