HRD Benefits On-Site Cancer Screenings FY27 Pilot Project

The Ohio Department of Administrative Services ("DAS") State Human Resources Division (“SHRD”) is issuing this Request for Quotes (“RFQ”) for the on-site skin cancer screenings described in the RFQ document that is in the document section of this solicitation.

Please be sure to include the state contract number in the contract column of the grid, if applicable.

Completion of the grid is required and will be considered as part of the response for evaluation.

Completion of the questionnaire is required and will be considered as part of the response for evaluation.

The State Standard Terms and Conditions apply to this solicitation (attached).

Selection will be based on the evaluation and scoring of the quetionnaire and the cost.

DAS may execute a written contract with the awarded supplier.

All responders to this solicitation must be fully registered as a Supplier to do business with the State of Ohio. Please follow all steps below to be fully registered.

At its sole discretion, DAS may renew the contract for up to three additional one-year term(s) subject to the satisfactory performance of the selected supplier and the needs of the State.
