NOTICE OF INTENT TO SOLE SOURCE

Houva NBUVB Full Body Phototherapy Booth w/removable Internal Sensors 48 NBUVB Lamps

The Department of Veterans Affairs, The Louis Stokes Cleveland Department of Veteran Affairs Medical Center (CLE VAMC), 10701 East Boulevard, Cleveland, Ohio 44106 intends to solicit and negotiate with brand name only under the authority of FAR 8.405-6(b)(1), Limited sources justification and approval.

The NAICS code is 335139 with a large business standard of 1,250 employees. The VA intends to solicit and negotiate with National Biological Corp., 23700 Mercantile RD., Beachwood, OH 44122-5900 for HOUVA NBUVB Full Body Phototherapy Booth w/removable Internal Sensors 48 NBUVB Lamps equipment.

The CLE VAMC intends to provide a Houva NBUVB Full Body Phototherapy Booth and associated equipment and services. This equipment replaces the existing phototherapy unit, that has reached the end-of-life cycle.

National Biological Corp. is the original equipment manufacturer (OEM), and sole distributor of the following items, which makes this a brand name only, sole source procurement under FAR 8.405-6(b)(1).
