We believe in our vision, that Tuolumne County can be a safe and thriving community where every person achieves optimal health. This Health Equity Mini-Grant Program is aimed at advancing community health and wellness by offering funding to support the efforts of groups or individuals who are dedicated to improving health equity in Tuolumne County.

Background

What is Health Equity?

Health Equity is achieved when every person has the opportunity to attain their full health potential, and no one is disadvantaged from achieving this potential because of their social position or other socially determined circumstances. In other words, Health Equity means that everyone has a fair and just opportunity to be as healthy as possible and achieving this requires removing obstacles to health such as poverty, discrimination, and their consequences – including powerlessness and lack of access to good jobs with fair pay, quality education, affordable housing, safe environments, and healthcare.

What are the Social Determinants of Health?

Social Determinants of Health (SDOH) are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. They are social, economic, and environmental factors which shape our health. The SDOH have a major impact on people’s health, well-being, and quality of life.
