I. BACKGROUND
The City of Lodi experienced an increase in the number of unsheltered individuals from 2019 to 2024.
Homelessness across San Joaquin County increased by 104% during this same period, and in Lodi,
the homeless population grew from 139 unsheltered individuals in 2019 to 262 in 2024, making up 9%
of the County's total homeless population in 2024. The City of Lodi has taken many steps to address
the challenges of homelessness in our community and has devoted significant resources toward this
effort, including the development of Lodi's first Temporary Emergency Shelter, a 40-unit transitional
housing project, and a four-unit permanent housing project. These efforts are the driving force behind
the decreased numbers in the 2026 Point-In-Time count. To address the issues surrounding
homelessness, the regional strategic plan was adopted by Lodi City Council in November 2020. One
of the goals of this strategic plan is to increase access and reduce barriers to homeless crisis
services. To accomplish this, Lodi has developed the Harmony Homes project, a permanent
supportive housing program limited to homeless Lodi residents referred primarily from social service
providers in Lodi.
The project site is located at 301 East Lodi Ave in Lodi, California, and identified as San Joaquin
County Assessor's Parcel Number 043-067-16. This project consists of one accessible and three
standard modular homes situated on a 10,581-square-foot lot with 160 feet of direct frontage. Each
house offers 490 square feet of living space with a covered porch, appliances (refrigerator, range,
water heater, and heater), utility access (electricity, water, sewer, and trash), and facilities (sink,
shower, and lavatory). The lot contains secure parking with an accessible electric gate, 4-foot
perimeter fence, and landscaping. Each unit will be occupied by a maximum of three inhabitants,
including, at minimum one, (1) adult homeless resident or such other number of homeless residents
as may be permitted by the Zoning Code in effect at the time. These individuals will have received a
Housing Choice Voucher (HCV) from the Housing Authority of the County of San Joaquin (HACSJ)
and will also need to pay no less than 30% of their monthly income (after exemptions) and no more
than 40% for lodging.
II. INTRODUCTION
The funding for the development of this project is from the City's general fund, Homeless Emergency
Aid Program ("HEAP") and Homeless, Housing Assistance, and Prevention ("HHAP") grant funds. The
general purpose of the Homeless Emergency Aid Program ("HEAP") is to provide direct assistance to
cities and counties to address the homelessness crisis throughout California. The general purpose of
the Homeless, Housing Assistance, and Prevention ("HHAP") grant funds is to provide a one-time
block grant funding to support regional coordination and to expand or develop local capacity to
address immediate homelessness challenges. Activities will be informed by a best-practices
framework focused on moving homeless individuals and families into permanent housing and
supporting the efforts of those individuals and families to maintain their permanent housing. The
service provider shall comply with all applicable federal, state, and local laws and regulations,
including any requirements associated with funding sources and Housing Choice Voucher
participation.
Target Population
Harmony Homes serves homeless Lodi residents, referred primarily from social service providers in
the City of Lodi. Each unit houses a maximum of three inhabitants, including at minimum one (1) adult

homeless resident who has received a Housing Choice Voucher (HCV) from the Housing Authority of
the County of San Joaquin (HACSJ). Tenants must pay no less than 30% and no more than 40% of
their monthly income (after exemptions) toward lodging. The Service Provider must demonstrate
experience serving individuals experiencing homelessness and be prepared to accommodate varying
levels of need, including residents who may also meet disability or income-based eligibility criteria
under HEAP, HHAP, or related programs.
Work Scope
The City of Lodi is looking to award a Permanent Supportive Housing Operational Management
Service Provider (hereinafter the "Service Provider") for this program. The Service Provider shall
demonstrate experience in permanent supportive housing programs and maintaining compliance with
applicable federal, state, and local requirements. The ideal Service Provider has experience
determining eligibility for individuals experiencing homelessness and meeting applicable disability and
income eligibility requirements, set forth by the Homeless Emergency Aid Program (HEAP),
Homeless, Housing Assistance, and Prevention ("HHAP"), Permanent Local Housing Allocation
Program (PLHA), California Business, Consumer Services, and Housing Agency (BCSH), California
Department of Housing and Community Development (HCD), U.S. Department of Housing and Urban
Development (HUD), Housing Authority of the County of San Joaquin (HACSJ), San Joaquin
Continuum of Care (SJCoC), and the City of Lodi.
The Service Provider will refer or receive referrals for clients in the City of Lodi and determine client
eligibility per federal and City standards. The Service Provider shall compile and submit all
documentation as needed to the Housing Authority of the County of San Joaquin for Housing Choice
Voucher Eligibility Determination. Rental income (client rent and any identified subsidies) and self-
procured grants must cover administration, facility management, day-to-day maintenance, janitorial,
on-site security, case management of clients, and utilities. The City will not be providing any additional
operating subsidies but will be responsible for deferred maintenance of major mechanical and facility
items. The Service Provider will be responsible for day-to-day maintenance of the property and minor
repairs. The property must be kept up to standards per the 2025 California Building Standards Code
(including the Building, Residential, Electrical, Plumbing, and Fire codes) alongside the City's locally
amended Municipal Code. The Service Provider shall ensure all tenants are receiving case
management, either by the referred agency or by the Service Provider. The Service Provider shall
conduct periodic check-ins with all tenants. The Service Provider shall adhere to and have experience
with fair housing standards and practices, as required by the federal government 24 CFR Part 100,
nondiscrimination practices as outlined in 24 CFR Part 1, and all other applicable federal, state, and
local regulations. The Service Provider shall maintain the residential areas drug-free, alcohol-free, and
smoke-free.
The Service Provider shall have the applicable staff available to provide all the necessary functions of
the service, including but not limited to:
* Providing quarterly reports to the City: Quarterly reports shall include all tenants currently
residing in homes, income eligibility determination documentation, rental income/expenses,
demographics, and tenancy outcomes, including voluntary departures and evictions.

* Accept and process applications for potential tenants, including intake forms, release of
information forms, leases and all other documentation needed as required by federal, state,
and City regulation.
* Interview prospective tenants, arranging tours, explaining, and maintaining tenant rules,
outlining methods of rental payments, and answering tenants' questions and concerns.
* Establish rental arrangements with tenants and HACSJ based on adjusted monthly income,
exemptions, and all other rules and regulations established by the federal and state
regulations.
* Establish methods of eviction boundaries and procedures.
* Assist in tenant resolutions when necessary.
* Maintain communal areas.
* Ensuring property maintenance per property standards - as well as household furnishings and
appliances, gardening/landscaping, business property, and liability insurance.
* Inspect units regularly for evidence of wear-and-tear, accidental and intentional damage, and
defective systems, and conducting repairs as appropriate.
* Prepare units for the arrival of new tenants when necessary.
* Provide ongoing, regular, in-home housing case management - "regular" case management
may vary, and can be monthly for some tenants and daily for others, especially if someone is
in crisis.
* Support clients in maintaining housing starting with initial contact, lease up, move in, renter
education, and benefits assistance.
* Connect clients to ongoing appropriate services for behavioral health, primary physical health
care, substance use recovery, peer support, and other community support when needed.
* The selected Service Provider will be required to enter client level data in the San Joaquin
County Homeless Management Information System (HMIS) for each resident, in accordance
with Continuum of Care requirements. The HMIS is a database used by the San Joaquin
County Continuum of Care (SJCoC) for the purpose of recording and storing client-level
information on the characteristics and service needs of those experiencing homelessness. To
participate in the SJCoC, Contributing HMIS Organizations (CHOs) must have an Agency
Partner Agreement and a Data Sharing Memorandum of Understanding with the SJCoC. For

The successful Service Provider will provide the City with a detailed management plan of how the
Harmony Homes Project will successfully run detailing, staff, administrative processes and
procedures, tenant rules and regulations, expectations from the Service Provider, the tenant and the
City, tenant lease, intake process, exit process, and all other provisions needed to run Harmony
Homes in compliance with federal, state, and City regulation.

The successful Applicant will be notified within a month of the closing date of the RFP. The initial term
of the Agreement shall be for 1 year with the option to extend an additional five years based upon
acceptable performance by the Service Provider. Any Applicant submitting a proposal will need to
demonstrate experience in housing navigation, property management, and case management for
individuals experiencing homelessness. Service Provider shall meet the RFP requirements and
demonstrate the ability to operate and maintain permanent supportive housing for those experiencing
homelessness in the Lodi community in order for the submission to be accepted.
The City reserves the right to request that Applicants submit additional information as may be
requested by staff to clarify or augment submitted information. Also, the City reserves the right to
suspend, amend, or modify the provisions of this RFP, to reject all proposals, and/or to negotiate
modifications of proposals.
Prerequisites
Proposals will only be considered from Applicants that meet the following prerequisites:
* The Applicant shall be a qualified private or public nonprofit organization currently engaged in
providing homeless services and successfully managing homeless service centers. Have a
minimum of three (3) consecutive years of successfully managing and operating homeless
programs and delivering relevant services of a similar type and scope as described in sections
III, IV, and V of this RFP, and the sample Lease, Regulatory, and Operator Agreement
(Attachment A).
* The Applicant shall keep all tenant files in a safe location. All documents and files must be
retained for a minimum of three (3) years.
* The Applicant shall have not filed for bankruptcy under any business name over the past five
(5) years.
* The Applicant shall be in good standing with the federal government (not debarred)
* The Applicant shall have the current organizational experience and staff capacity to operate
permanent supportive housing.
* The Applicant shall employ key personnel experienced in case management services -
resumes will be requested
* The Applicant shall comply with grant funding laws and regulations and have the operational
capacity to do so. These requirements may include federal record-keeping, reporting and
financial requirements under 24 CFR Part 570 and 2 CFR Part 200 and record keeping and
data collection that may be required by the City and County.
* The Applicant shall be able to perform all the requirements as outlined within the RFP and
meet the requirements of the sample Lease, Regulatory, and Operator Agreement
(Attachment A), which is subject to change at the City's sole discretion

City's Rights
The City's rights include, but are not limited to, the following:
1. Issuing addenda to the RFP, including extending or revising the timeline for submittals.
2. Withdrawing, re-issuing, or modifying the RFP.
3. Requesting clarification and/or additional information from an Applicant at any point in the
process.
4. Committing or offering funding to one or multiple Applicants.
5. Executing an Agreement with an Applicant(s) based on the original and/or other information
submitted by the Applicant during the procurement process.
6. Rejecting any or all proposals, waiving irregularities, accepting, or rejecting all or any part of
any proposal, waiving any requirements of the RFP, as may be deemed in the best interest of
the City.
Applicant's Responsibilities
It is the responsibility of each applicant to:
1. Examine this RFP thoroughly, including all exhibits and attachments.
2. Become familiar with local conditions that may affect cost, performance, or services.
3. Applicant must use mail, email, or other delivery mechanisms at their own risk, and the City
shall not be obligated to accept or respond to any submission that is delayed due to delivery
failures.
Submission Requirements
Proposals must be submitted electronically as a single PDF file via email to
NeighborhoodServices@lodi.gov no later than August 20, 2026, by 5 p.m. Proposals submitted after
this deadline, regardless of cause, will not be considered. The City is not responsible for proposals
lost, delayed, or misdirected due to technical or transmission errors. Applicants are encouraged to
submit well in advance of the deadline and to request confirmation of receipt. The email subject line
must read: "Request for Proposal for Permanent Supportive Housing Operational Management
Services - [Organization Name]."
The proposal PDF must be assembled in the following order and must not exceed 25 pages,
excluding attachments and the budget:
1. Cover letter (signed by an individual authorized to bind the organization)
2. Applicant Information (Section IV)
3. Proposal Requirements and Narrative responses, numbered to correspond with Section V
4. Line-item budget (Part G under Section V)

The City reserves the right to deem non-conforming proposals non-responsive.
Consequence of Submission of Proposal
The submission of a proposal shall not be deemed as an agreement between the Applicant and the
City. The proposal is an offer by the Applicant to perform services in accordance with the proposal.
The proposals received shall become the exclusive property of the City.
All documents submitted in response to the RFP are considered public records and will be made
available to the public upon request, unless exempt from disclosure under the Public Records Act or
other applicable law. Please do not include confidential information or information that may violate the
privacy or intellectual property rights of a third party.
Cost of Submitting Proposals
The cost of preparing and submitting a proposal is the sole responsibility of the Applicant and shall not
be chargeable in any manner to the City. The City will not reimburse any Applicant for any costs
associated with the preparation and submission of a proposal, including but not limited to, expenses
incurred in making an oral presentation or participating in an interview.
Period of Performance
The initial term of the Agreement shall be for one year from the date that the Agreement is fully
executed with the option to extend for five years based upon acceptable performance by the
Successful Applicant and subject to the same terms and conditions of the Agreement.
The Agreement and all amendments will be subject to City Council approval.
III. PROPOSAL OBJECTIVES and REQUIREMENTS
Applicants must demonstrate the experience, capacity, and operational approach necessary to
manage Harmony Homes in a manner consistent with the funding, eligibility, and compliance
requirements described in Sections I and II. At a minimum, a responsive proposal must show the
ability to:
* Operate within the self-sustaining rent-and-subsidy model described in the Work
Scope, without additional City operating subsidy;
* Meet the eligibility, referral, and case management requirements of HEAP, HHAP,
HACSJ, and the applicable federal and state housing authorities; and
* Fulfill the applicable staffing, reporting, and compliance functions described in the Work
Scope.
Applicant Information is requested in Section IV. Proposals will be evaluated against the criteria and
narrative questions set out in Section V, scored according to the rubric in Section VII.

V. PROPOSAL REQUIREMENTS AND NARRATIVE
Applicants must respond to all questions below, organized by the evaluation category. Limit
responses to 500 words per numbered question unless otherwise noted.
A. Organizational Overview and Experience (20 points)
1. Provide a summary of the proposed services to be provided (work to be performed; expected
outcomes to be achieved).
2. Describe your experience in performing activities to those proposed in the RFP, including a
brief description of work performed and outcomes. If the scope of work performed is new to
the organization, please describe relevant experience that will allow your organization to
perform the scope of work successfully.
3. Describe your organization's history providing services to the homeless community, including
current and past collaborations with other nonprofits, faith-based organizations, agencies, and
other community stakeholders.
B. Staffing Plan and Capacity (20 points)
4. Describe your proposed staffing plan, including key positions, supervision structure, and staff
qualifications. Resumes for key personnel should be included.
5. Describe your organization's capacity to provide on-site coverage and periodic tenant check-
ins.
C. Service Delivery and Operational Methodology (20 points)
6. Provide a high-level summary of your methodology for operating the Harmony Homes project,
and specify the tasks/activities to accomplish program objectives.
7. Discuss your capability to develop, implement, and administer permanent supportive housing.
Provide proposed admission criteria and operational procedures, including client
rules/guidelines; identification requirements; resident eligibility criteria; method for determining

eligibility and rent; referral acceptance process; new resident orientation; case management;
and exit process (voluntary departure and eviction procedures).
8. Describe how clients will be connected to social services and support needed to maintain
stable, permanent tenancy.
9. Share your organization's participation and history with the California Homeless Management
Information System (HMIS) and the San Joaquin Continuum of Care's coordinated entry
system, including your plan for HMIS compliance, your recordkeeping and data-sharing
practices, and the software used to track demographics and statistical data.
10. Confirm whether your organization has completed and signed California's (CalHHS) Data
Sharing Framework Data Sharing Agreement to perform member matching and sharing
information on housing data? If not, do you intend to, and if so, what is your estimated
timeline?

D. Community Coordination and Good Neighbor Plan (10 points)

concerns, minimizing impacts to surrounding neighborhoods and businesses, etc.
