REQUEST FOR PREQUALIFICATION
OF CONTRACTORS
1. GENERAL: The Department of Finance and Administration's Bureau of Building, Grounds and
Real Property Management (hereinafter "Bureau of Building") will contract for construction of the
projects listed in this document. This is NOT a request for bids/pricing. Contractors who are
prequalified under this process will be requested to submit sealed bid in a separately issued
Invitation for Bids. Qualifications shall be submitted in a sealed envelope with the GS# and
name of the project clearly indicated on the outside of the envelope or electronically via
MAGIC to RFx Number 3160008094. Physical submissions shall include one (1) hard-
copy of all information required and five (5) flash drives. Electronic submissions shall
include all information required except for the required hard-copy and AIA Document
A305-2020 with Exhibit B Financial and Performance Information for the Contactor which
may be submitted in hard-copy format not later than twenty-four (24) hours of the
submission date and time.

Scope of work includes sitework and construction of a new one-story, 170,000+/- square
foot Nursing Home designed to serve the special needs of 150 residents with private
bedrooms and baths. The facility also includes common dining and living rooms, bathing
suites, therapeutic areas, administrative offices, barber shop, commercial kitchen, laundry,
maintenance, and other support spaces. The foundation system is composed of grade beams
with interconnected slab-on-grade. The roof framing system consists of prefabricated cold-

3. SELECTION PROCESS: A Selection Committee will review all proposals submitted
for each project and establish a list of all responsible and responsive proposals meeting a
minimum score of 70 points. The Selection Committee for each project will consist of
three (3) representatives of DFA and two (2) representatives of the listed Using Agency.
The Selection Committee may elect to further refine the list to not less than two (2) and no
more than five (5) Contractors receiving the highest scores. Interviews, if held, are open
to other representatives of DFA, the Using Agency or Governing Board; however, they will
not participate in any re-scoring following such interviews. Interviews, if held, will be
scheduled the week of 07/20/2026. Evaluation criteria for selection for this RFP shall
be as follows (100-point scale):
1. General Contractor Past Performance & Relevant Experience (35 Points):
Scoring for this criteria will be based upon references provided and/or consultation
with Owner(s) of completed reference projects identified in the Contractor's
submission. Performance shall be documented in the format stipulated herein a
minimum of five (5) reference projects that are on-going contracts or contracts
completed not more than three (3) years ago that demonstrate ability to perform work
of the size, character, and complexity of the project being solicited. All information
received from those clients must confirm a minimum rating of SATISFACTORY to be
eligible for award of project. No relevant contracts within the identified period shall
be excluded. All relevant contracts, whether federal, state, or local or for private sector
clients will be evaluated equally. Failure to provide documents that would impact
performance rating of Contractor may be considered non-responsive.

2. Project Management Team (10 Points): General qualifications and experience of
office and field management personnel identified by General Contractor, award points
depending upon resume and personal references. Contractor shall identify and provide
resumes and references for key management and construction personnel proposed for
the prosecution of the work. It is the expectation of the Owner that personnel identified
will be retained in such roles for the duration of the project.
3. Key Sub-Contractor Past Performance & Relevant Experience (30 Points):
Qualifications and experience of Key Sub-Contractors identified in the Contractor's
submission. Where more than one Key Sub-Contractor is identified for each role, score
will be based upon the least qualified/experienced Key Sub-Contractor identified.
Performance for each Key Sub-Contractor proposed shall be documented in the format
stipulated herein a minimum of five (5) reference projects that are on-going contracts
or contracts completed not more than three (3) years ago that demonstrate ability to
perform work of the size, character, and complexity of the project being solicited. All
