The vendor will be held to the requirements of F.O.B. Destination as set forth in the
Commonwealth of Pennsylvania Terms and Conditions. The vendor may quote shipping
costs, but it must be listed on a separate line item in the quote. The awarded vendor will
be paid for the total line items on the quote.
Additional Requirements:
* Please provide estimated shipping lead-time.
* Complete and attach GSPUR-89 - Reciprocal Limitations Act Requirements (Required for orders
exceeding $10,000.00) with bid.
* Once award is made, we will require the awarded vendor to come to the end users' location in
Lewistown, PA to size each individual for exact size measurement. The sizes listed on this
specification are for general pricing in the event sizing changes the unit costs. If different sizes
are required, and price changes, the purchase order can be adjusted to accommodate these
price changes. If a fee is required for sizing, please add to the bidder remarks in the IFB.

Substitutions Allowable:
Substitution with an approved equal item is allowable.
Any reference to a particular manufacturer's product either by trade name or by limited description is solely for
the purpose of more clearly indicating the minimum standard of quality required.
The term "approved equal" is defined as meaning any other make which, in the sole opinion of the purchaser and
the end recipient/subject matter expert (SME), is of such character, quality, and performance equivalence as to
meet the standard of quality of products other than the referenced product shall:
a) furnish complete identification in its quote of the product it is offering by trade name, brand and/or
model number;
b) furnish descriptive literature and data with respect to the substitute product it proposes to furnish; and
c) indicate any known specifications deviations from the referenced product.
