This Addendum supplements and modifies the Project Manual and Drawings (Construction
Documents) for the above Project, and shall hereby be incorporated into the bid and be made part
of the Contract Documents. Bidders shall verify and acknowledge this fact by indicating receipt of
the Addendum on their Bid Form.
Attachments:
1. Revised Specification Section 004113 – Bid Form. 8 pages, included with Addendum file.
Revisions to Specifications (revisions noted in bold text):
1. Specification 004113 – Bid Form (specification re-issued, see attachment):
• Delete the original Bid Form in its entirety and replace with the attached Bid
Form.
o Unit price allowances corrected to match the specification section
012100 and Addendum No. 1 modifications.
Clarification
• The bid due date is unchanged. Bids are due Wednesday, May 1, 2024 at 2:00 pm. The
pre-bid meeting minutes issued with addendum No. 1 had conflicting information. The date
in the meeting minutes should be disregarded.

Sealed bids will be received by Miami University’s Physical Facility Department at 101 Fisher Drive, Oxford Ohio 45056
and read aloud upon opening at the Physical Facilities Department and via the Ionwave portal (as detailed and set forth in this
Project’s Solicitation) for:

2024 Garages Repairs
Bidder acknowledges that by signing the Bid Form on the Bidder Signature and Information page, that it has read and
understands the applicable Executive Orders regarding the prohibitions of performance of offshore services, locating State
data offshore in any way, or purchasing from Russian institutions or companies. If awarded a Contract, the Bidder will
become the Contractor and affirms that both the Contractor and its Subcontractors shall perform no services requested under
this Contract outside of the United States.
The Bidder shall provide the locations where services under this Contract will be performed in the spaces provided below or
by attachment. Failure to provide this information as part of its Bid may cause the Bidder to be deemed non-responsive and
no further consideration will be given to its Bid. If the Bidder will not be using Subcontractors, indicate “Not Applicable” in
