HEALTH CARE FOR HOMELESS VETERANS (HCHV) SERVICES

To provide HCHV Services as described in the attached

Performance Work Statement. The period of performance is

one (1) base year with four (4) one-year option periods.

This is a firm-fixed price Indefinite Delivery/Indefinite

Quantity solicitation.

Services are being solicited for the St. Louis VA Medical

Center John Cochran Division with the intent for multiple

awards.
