Summary

Central Ohio Behavioral Healthcare is soliciting bids for a contractor to perform maintenance on our kitchen equipment.

Pre-bid walkthrough is scheduled on 05/21/25 at 9:00am:
Address is 2200 West Broad Street, Columbus, OH  43223 in the main (new) building.

SCOPE OF WORK AND DELIVERABLES

Work is to be performed within State Fiscal Year 2026 – July 1, 2025 through June 30, 2026 and State Fiscal Year 2027 – July 1, 2026 through June 30, 2027.

Hobart Dishwasher requires a service agreement to include the following:
  1. 24/7 parts, labor, and travel
  2. 4-hour emergency response/24-hour non-emergency response
  3. Two inspections (March and September) per year
  4. Two PM (replace wear parts) per year June and December
  5. Parts coverage and expedited parts coverage
  6. Unlimited service calls
  7. Maintaining Records/Reporting
Services included:
  1. During the Term, Contractor will perform repair services on the listed equipment Monday through Sunday, 24-hours day, based on Calls received from Customer, in accordance with the Call types and response times specified below. All labor and parts (including shipping) necessary for such repair services (except as otherwise provided herein) are included. Unlimited number of Calls per Term.

Itemized Quote: Please provide an itemized Quote when submitting your bid. The quote package shall include your technical proposal with experience levels.

Controlling Board Threshold: In accordance with Ohio Revised Code 127.162, this solicitation may be subject to Controlling Board threshold. Bids received will be reviewed against our Agency’s Controlling Board threshold. Suppliers exceeding our Agency’s Controlling Board threshold may prevent us from awarding a contract to your organization.

Evaluation Process


OhioMHAS’s evaluation process of responses submitted to this request may consist of up to four distinct phases:
  1. OhioMHAS’s initial review of all proposals for timely submission;
  2. An evaluation committee review of the proposals for defects and scoring;
  3. OhioMHAS’s request for more information (clarifications, interviews, presentations, and/or demonstrations); and,
  4. Negotiations or best offer requests.


At its sole discretion, OhioMHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. OhioMHAS may add or remove sub-phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.

Proposal Evaluation Criteria

In the proposal evaluation phase, OhioMHAS staff or reviewers selected by OhioMHAS (the committee) will rate the proposals submitted in response to this RFP based on the following criteria and weight assigned to each criterion.
