FUMIGATION FOR TERMITE/PESTS CONTROL AT THE KONA CES BUILDING

Project Description: Fumigation for termite/pests control at the Kona CES Building. Address: 79-7381
Mamalahoa Highway; Kealakekua, Hawaii. Termite tenting and fumigation per attached specifications.

Special Instructions

1) Price to include tax and service charge to University of Hawaii, Kona Extension
Service, 79-7381 Mamalahoa Highway; Kealakekua, HI 96750.

2) Certificate of insurance required prior to service and must include University of
Hawaii as additionally insured, waiver of subrogation in favor of UH for all policies listed, and liability
coverage must be primary and non-contributory where required by written contract -
see requirement.

3) Vendors shall obtain a copy of their \\\"Certificate of Vendor Compliance\\\" via
Hawaii Compliance Express (https://vendors.ehawaii.gov/hce/splash/welcome.html). If
not registered/complaint already it is strongly advised to register and submit
compliance documents as soon as possible to avoid delays.

4) Vendors who have not done business with the University of Hawaii, or have not
done business after July 1, 2012, may be required to submit a W-9 tax document per
University policy.

5) It is the responsibility of all bidders to review the attachments to this Request
for Quotation. Submission of a bid confirms a bidder has carefully examined these
documents and agrees to the University of Hawaii\\\'s terms and conditions in place of
Vendor\\\'s own terms and conditions.

Subject to prior approval if making substitutes, we reserve the right to determine
equivalency. Email with specs if making substitutes or if you have questions.

7) Vendor who is awarded this RFQ must submit required COI, signed award, certificate of tax compliance
and any other required documents within 7 business days of the email notification of the award. Any
delay can be negotiated, though the intent is for UH to request a PO as soon as possible after the
award.
