Period of coverage starting day of PO issuance for 12 month.

Work to include the lubrication and operational preventative maintenance (PM) inspection, performed by a certified technician, bi-annually with four (4) material handling pieces of equipment located at 50 W. Town Street and 2080 - 2088 Integrity Drive N. as suggested by the manufacturer’s suggested service manual. Preventative maintenance to include as applicable by machine, Filter, Oil, Grease, Anti-Freeze, Hydraulic Oil, Tires, Radiator water , Battery Condition, Horn, Dashboard Instruments, Lights, Steering Mechanism, Brakes (running and parking), Leaks, Wheel nuts/bolts, Masts Chains and Forks and Seat belts. Inspection report to be provided after each preventative maintenance evaluation and service repair.

Repair work outside of the PM service is to be performed at the labor hourly rate with OEM parts to be quoted at actual cost to the program manager for approval to proceed with the repair.

The State of Ohio - Department of Administrative Services (DAS) is soliciting for Forklift Maintenance.

Please be sure to include the state contract number in the contract column of the grid, if applicable.

Completion of the grid is required and will be considered the response for evaluation.  No outside/additional documentation will be considered.

The State Standard Terms and Conditions apply to this solicitation.  No additional terms and conditions will be accepted.

Selection will be based on the lowest responsive and responsible quote.

DAS may execute a written contract with the awarded supplier.

All responders to this solicitation must be fully registered as a Supplier to do business with the State of Ohio.  Please follow all steps below to be fully registered.

At its sole discretion, DAS may renew these services for up to four years.
