This RFP is intended for use as a Food Service Management Company Contract
Assurance of Civil Rights Compliance (NSLP/SBP)
The program participant hereby agrees that it will comply with:
Title VI of the Civil Rights Act of 1964 (42 U.S.C. 2000d et seq.);
Title IX of the Education Amendments of 1972 (20 U.S.C. 1681 et seq.);
Section 504 of the Rehabilitation Act of 1973 (29 U.S.C. 794);
Age Discrimination Act of 1975 (42 U.S.C. 6101 et seq.);
All provisions required by the implementing regulations of the Department of Agriculture;
Department of Justice Enforcement Guidelines, 28 CFR Part SO.3 and 42;
FNS directives and guidelines, to the effect that, no person shall, on the grounds of race, color, national origin, sex,
age, or disability, be excluded from participation in, be denied benefits of, or otherwise be subject to discrimination
under any program or activity for which the program applicant receives Federal financial assistance from FNS; and
hereby gives assurance that it will immediately take measures necessary to effectuate this agreement.
By accepting this assurance, the Program applicant agrees to compile data, maintain records, and submit reports as
required, to permit effective enforcement of nondiscrimination laws and permit authorized USDA personnel during hours
of program operation to review such records, books, and accounts as needed to ascertain compliance with the
nondiscrimination laws. If there are any violations of this assurance, the Department of Agriculture, FNS, shall have

Assurance of Civil Rights Compliance (CACFP/SFSP)
The program participant hereby agrees that it will comply with:

by the regulations of the Department of Agriculture (7 CFR Part 15);
DOJ (28) CFR Parts 42 and 50) and FNS directives or regulations issued pursuant to that Act and the regulations,
to the effect that, no person in the United States shall, on the ground of race, color, national origin, sex, age, or
disability, be excluded from participation in, be denied the benefits of, or be otherwise subject to discrimination
under any program or activity for which the Program applicant received Federal financial assistance from USDA;
and hereby gives assurance that it will immediately take any measures necessary to fulfill this agreement.
This assurance is given in consideration of and for the purpose of obtaining any and all Federal financial assistance,
grants, and loans of Federal funds, reimbursable expenditures, grant, or donation of Federal property and interest

General Information
A. Intent
This Request for Proposal is for the purpose of entering into a contract for the operation of a food service program
for Southern University Laboratory School (Name of SFA/Sponsor), hereinafter referred to as the School
Food Authority (SFA)/Sponsor.
The Food Service Management Company will be referred to as the FSMC, and the contract will be between the
FSMC and the SFA/Sponsor.
B. Procurement Method:
Procurement shall be executed through competitive proposals. Competitive proposals involve the use of a
solicitation document known as a Request for Proposal (RFP).
All procurement transactions shall be conducted in a manner that provides maximum open and free competition
consistent with 2 CFR Part 200.318-327 and Appendix II to Part 200.
C. Pre- Proposal Meeting
A meeting with interested FSMCs to review the specifications, to clarify any questions, and for a walk-through
of the facilities with school officials will be on April 28, 2026 (Date) at Southern University Laboratory School
(Location) at 8:30 (Time) am/ pm (CST)
Attendance is required optional. Vendor presentations will will not be scheduled at this time.
D. Proposal Submission and Award
SFAs/Sponsors must use the prototype FSMC Request for Proposal/Contract. An SFA/Sponsor not completing
the required procurement procedures will not be approved for participation in a reimbursement program. The
SFA/Sponsor must complete all of the required information in the RFP. Failure to do so will prevent the
SFA/Sponsor from awarding the contract.
All FSMCs submitting a proposal must complete all of the information required in the RFP. The completed RFP
must be returned to the SFA/Sponsor from the FSMC as a part of the proposal. Proposals received
without the completed RFP documents will be considered non-responsive and the FSMC's proposal will
be disqualified.
Each prospective FSMC must provide a sealed copy of the entire proposal through the means set forth by the
SFA/Sponsor. The SFA/Sponsor shall provide an electronic copy of each proposal to the State Agency.
Federal and state regulations allow contracts to be a duration no longer than one year, with the option of yearly
renewals not to exceed four additional years (7 CFR Part 210.16(d) and L.R.S. 39:1615.A). If the SFA/Sponsor
chooses to renew the contract, the Attestation/Agreement Amendment document must be submitted to the state
