SPACER, FOAM 5 INCH DIAMETER X 3/4 INCH THICKNESS. MATERIAL IS POLYETHELENE MATERIAL, CLASS 4, GRADE B, TYPE I. MUST MEET ALL REQUIREMENTS OF SPEC A-A-59136 (28 OCT 97) FOR CLASS, GRADE AND TYPE, CERTIFICATE OF CONFORMANCE AND SDS REQUIRED WITH EACH SHIPMENT, ETHAFOAM 220 OR EQUAL.         COC/COA MUST INCLUDE MANUFACTURER NAME, PRODUCT NAME/DESCRIPTION, SPECIFICATION, LOT/BATCH NUMBER, MANUFACTURE/EXPIRATION DATE, ANALYTICAL DATA, STATEMENT OF NO CHANGE IN MATERIALS OR PROCESSES, AND SIGNATURE OF AUTHORIZED REPRESENTATIVE.PACKAGING LABELS WILL INCLUDE MANUFACTURER NAME, PRODUCT NAME/DESCRIPTION, LOT/BATCH NUMBER AND MANUFACTURE/EXPIRATION DATE.COC/COA APPROVAL BY MCAAP AO REQUIRED BEFORE CONTRACT AWARD.REQUIRED DELIVERY ON OR BEFORE AUGUST 5, 2026
