Solicitation HT940625R0008 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attached Standard Form (SF) 1449. The SF1449 form is being utilized to generate the applicable clauses from the Standard Procurement System.
The Defense Health Agency Contracting Activity, Healthcare Contracting Division - Southeast intends to procure a firm fixed price contract for Fiber Optic Cable Service in support of Naval Hospital Beaufort. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform communication capabilities. The contractor shall continue to maintain and service the current infrastructure for dial tone and signaling services through fiber optics for Naval Hospital Beaufort. The contract will consist of a base period and four option periods (12 months) with a 6-month option. This requirement is being solicited in accordance with Federal Acquisition Regulation Part 13 – Simplified Acquisition Procedures.
Competition Approach: Full and Open Competition
Submission of Proposals: Contractors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Proposal Instructions and Evaluation Criterion provided in the solicitation document to create the proposal. Proposal is required to be submitted electronically to the Contracting Officer and Contract Specialists on or before the date and time specified in SF 1449, Block 8 of Page l.
Period of Performance: Base Period – 1 May 2025 – 30 April 2026
Option Period 1 - 1 May 2026 – 30 April 2027
Option Period 2 - 1 May 2027 – 30 April 2028
Option Period 3 - 1 May 2028 – 30 April 2029
Option Period 4 - 1 May 2029 – 30 April 2030
Option Period 5 (-8) – 1 May 2030 – 31 October 2030
