EXT HSA | RFP # 1175 Home-Delivered Grocery Prog

The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announce their intent to seek proposals from nonprofit organizations interested in providing a home-delivered grocery (HDG) program for older adults and adults with disabilities residing in the City and County of San Francisco.

DAS supports a variety of multimodal home-delivered grocery (HDG) program models to address the diverse needs of eligible consumers and has allocated funding for program models based on current service provision, the current Dignity Fund Community Needs Assessment, and the Service and Allocation Plan for FY 2023-24 through FY 2026-27

This procurement is for existing programming. The agencies awarded new grants will be required to collaborate with DAS in developing a comprehensive transition plan as needed. This plan is crucial to ensuring the continuity of services for current consumers when the new grant begins. The successful bidder(s) will work closely with DAS to facilitate a smooth transition, minimizing any disruptions to service delivery during this period.
