TECHNICAL
Qualifications and Experience
1. Describe your experience evaluating complex, multi-partner initiatives. Include examples in
healthcare, public health, Medicaid, rural health, transformation efforts, and related
systems.
2. Explain your experience analyzing both quantitative and qualitative data. What methods
and tools do you use? How do you translate findings into actionable, practical
recommendations for stakeholders? Provide examples.
3. Provide examples of how you have supported continuous improvement efforts using
evaluation findings to inform mid-course corrections and real-time decision-making.
4. Describe your experience developing clear, compelling materials that communicate project
success, outcomes, and lessons learned. Describe your experience with graphic design.
Include examples of materials created for different audiences.
5. Describe your experience working with state and federal funders and grant-funded work. In
particular, please expand on any experience working with New Hampshire-based
organizations. Include any evaluation and performance measurement experience with the
Centers for Medicare & Medicaid Services (CMS).
Technical approach
1. Describe your proposed approach to this project, including evaluation design, performance
monitoring, mid-course correction, implementation science and learning, and
sustainability.
2. Describe how you will support GO-NORTH in promoting transparency into performance,
progress, barriers, and outcomes.
3. Describe your proposed approach to developing easy-to-use performance monitoring
tools.
4. Describe your proposed approach to developing materials that communicate success,
impact, and lessons learned.
Collaboration and support
1. Describe your approach to partnering with GO-NORTH and diverse stakeholders while
maintaining objectivity as a third-party evaluator. If applicable, describe how your
organization will respond to any perceived conflicts of interest.

2. Describe your approach to supporting contracted Hub organizations and other
implementation partners (government, healthcare, public health, community based
organizations) in using findings to improve performance.
3.Describe your organization's ability to perform on-site support. Your response should
include the locations of your physical office, field teams or service hubs relevant to the
engagement; your standard response times for onsite deployment after requests or incident
notification; the process for mobilizing personnel, including escalation procedures and after-
hours/weekend support; and the number and roles of staff available for immediate or short-
notice deployment.
3. Describe how you will tailor your approach to partners with varying levels of data capacity
and evaluation readiness with the goal of building sustainable systems for data collection,
reporting, and evaluation.
Staffing and management
1. Provide a proposed organizational chart for this project and detailed staffing information for
all proposed personnel and planned positions. Identify the project lead and key staff, and
describe their roles on this project. As detailed in the RFP, this must include resumes for all
key staff.
2. Describe your approach to maintaining staffing continuity and managing turnover during a
multi-year engagement.
3. Provide a proposed work plan for the first 12 months of the contract, including key
milestones, meetings, and deliverables.
If you plan to sub-contract or hire consultants for this project, describe your plans and how it will
benefit this project. Describe how you will ensure your team has the capacity to provide oversight
of any proposed sub-contractors or consultants.
