ESG and CDBG Homeless Services Funds

The Program Year (PY) 2025 Community Development Homeless Services application is available online through the Department of Community Development’s PowerApps grants management portal.

Applications must be submitted to the PowerApps portal by October 31, 2025 at 5:00 PM. Eligible organizations may use this single application to request the Emergency Solutions Grant (ESG) and Community Development Block Grant (CDBG) homeless services funds. These funds support the following services:

  • Street Outreach – engagement and assessment services for persons experiencing unsheltered homelessness
  • Emergency Shelter – safe and secure facility-based crisis housing and services
  • Rapid Rehousing – rental subsidy, housing case management, and supportive services for households experiencing homelessness
  • Coordinated Intake – the single point of entry into the Cuyahoga County homeless services system
  • Homeless Prevention – assistance to help imminently homeless households maintain or regain housing stability

Late submissions will not be accepted.

To qualify for funding, organizations must be in operation at least two years and all proposed activities must align with the goals of the Cleveland/Cuyahoga County Continuum of Care Strategic Action Plan to End Homelessness. Programs must also comply with Continuum of Care standards and participate in HMIS and Coordinated Intake.

ESG-funded programs require a dollar-for-dollar match. There is no match requirement for CDBG homeless services grants.

Applications will be scored on the program’s effect on homeless individuals and families, agency financial and programmatic capacity, collaboration with other providers, and past performance on Community Development contracts.
