I. Overview
The State of Delaware Department of Health and Human Services seeks professional services
for Enhancement and Maintenance of the Cancer and Chronic Disease Data Management
System
. This request for proposals ("RFP") is issued pursuant to 29 Del. C. 6981 and 6982.
The proposed schedule of events subject to the RFP is outlined below:

Each proposal must be accompanied by a transmittal letter which briefly summarizes the
proposing firm's interest in providing the required professional services. The transmittal letter
must also clearly state and justify any exceptions to the requirements of the RFP which the
applicant may have taken in presenting the proposal. (Applicant exceptions must also be
recorded on Attachment 4).
Furthermore, the transmittal letter must attest to the fact, at a minimum, that the Vendor shall
not store or transfer non-public State of Delaware data outside of the United States.
For technology related solicitations, Vendors may refer to the Delaware Department of
Technology and Information identified terms and conditions included in this solicitation.
The State of Delaware reserves the right to deny any and all exceptions taken to the RFP
requirements.

II. Scope of Services
A. Background
Delaware Division of Public Health (DPH) is a nationally accredited State Public Health
agency, recognized by the Public Health Accreditation Board (PHAB) for demonstrating
commitment to quality improvement, performance management, accountability,
transparency, and ability to address public health needs. DPH serves as the center of a
comprehensive network of public health services, establishing public health priorities and
evaluating the effectiveness of health initiatives.
The mission of the DPH is to protect and enhance the health of the people of Delaware.
DPH accomplishes its mission by:
* Working together with others;
* Addressing issues that affect the health of Delawareans;
* Keeping track of the State's health;
* Promoting positive lifestyles;
* Responding to critical health issues and disasters; and
* Promoting the availability of health services.
Bureau of Cancer Prevention and Control (BCPC) in Delaware
The Bureau of Cancer Prevention and Control (BCPC) and Bureau of Chronic Disease
(BCD) are ideally positioned in the Health Promotion and Disease Prevention (HPDP)
Section of the Delaware Division of Public Health (DPH). The BCPC programs provide
administrative staff support for the Delaware Cancer Consortium (DCC) and affiliated
committees. The COVID-19 pandemic unveiled the depth of health inequities in the U.S.
Low-income communities and Delaware's most vulnerable populations experience a
disproportionate burden of coronavirus in terms of both health outcomes and economic
impact. Through this data modernization project, the BCPC and BCD aims to improve
data accuracy and utilize its cancer and chronic disease data to inform program activities
and to conduct analyses to inform DPH leadership, healthcare providers, and other
stakeholders. Bidders of this RFP must propose equity focused data collection,
monitoring, and visualization strategies to support the achievement of the following
cancer prevention and control, and chronic disease management goals: eliminate
preventable cancers, ensure all people get the right screening at the right time for the
best outcome and support cancer survivors in a manner that allows them to live longer

heathier lives, track and manage patient information for the Chronic Disease Self-
Management Program, Healthy Behavior Support Service Programs, WISEWOMAN
Program, and the Emergency Medical Diabetes Fund.
The BCPC houses three components of CDC's National Cancer Prevention and Control
Program grants within one bureau:
National Program of Cancer Registries (NPCR)/Delaware Cancer Registry (DCR)

National Comprehensive Cancer Control Program (NCCCP)/Comprehensive

The BCPC also houses two primarily state-funded programs:

The programs within BCPC and BCD, although distinct in focus and outcomes, represent
individual components of the same Health Promotion and Disease Prevention team. The
Delaware Cancer Consortium (DCC) is a Governor-appointed consortium and focused
on reducing cancer incidence and mortality through evidence-base programming in a
multitude of areas. The DCC develops and oversees the implementation of the State
Cancer Plan which encompasses cancer screening and early detection, tobacco cessation
and physical activity awareness, environmental risk reduction (Healthy Homes),
improvement in the quality of cancer screening and care, the Delaware Cancer Treatment
Program, disparities elimination initiatives and services for people with cancer. The DCC
and state cancer goals and objectives can be found at the this weblink Next Five-Year

The Delaware Screening for Life (SFL) Program serves as the implementation vehicle
for the National Breast and Cervical Cancer Early Detection Program (NBCCEDP). The
SFL program was established in 1997 and offers dual eligibility determination and
enrollment into the SFL Program and/or the Health Care Connection (HCC) program. SFL
provides preventive cancer screening services to eligible clients. SFL utilizes a blend of
state and federal funding to provide lung, colorectal, and prostate cancer screening
services. SFL also provides care coordination to all SFL applicants to assist with mitigating
barriers to screening.
The Comprehensive Cancer Control Program (CCCP) implements and supports
cancer coalition efforts that leverage resources to plan and implement evidence-based

strategies to promote strategies focused on three primary priorities that directly align with
Delaware's 5-year State Cancer Control Plan:
* Priority 1: Primary Prevention
* Priority 2: Early Detection and Screening
* Priority 3: Health and Wellbeing of Cancer Survivors
The Delaware Cancer Registry (DCR) collects information about new cancer cases,
cancer treatment and cancer deaths. Registry data is used to monitor cancer trends,
promote research to increase survival, guide program and policy planning, and respond
to Delaware residents' cancer concerns.
The Health Care Connection (HCC) connects uninsured eligible clients with a medical
home and affordable healthcare services through the Medical Society of Delaware's VIP
provider network. Cancer screening services are specific to Screening for Life (SFL).
Since its initiation, SFL has provided clients with access to breast, cervical, colorectal,
lung and prostate cancer screenings and limited diagnostic services.
The Delaware Cancer Treatment Program (DCTP) was created and codified in 2004 to
provide insurance coverage to uninsured and underinsured Delawareans for up to two
full years of cancer treatment. DCTP serves as a payer of last resort and provides
benefits to the extent that services are not covered by another payer.
The Delaware Self-Management Program (SMP) is designed for adults aged 18 or
older who have or care for someone with diabetes, cancer, chronic pain, and/or multiple
chronic conditions. Workshops are two and a half hour sessions, once a week, for six
weeks, virtual or in-person.
The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-
BPSM) is a four-month, virtual program that focuses on self-monitoring blood pressure
with individualized support and teaches lifestyle techniques through nutrition education
and cooking classes.
The Well-Integrated Screening and Evaluation for Women Across the Nation
(WISEWOMAN) Program serves to reduce or eliminate the risk factors associated with
cardiovascular disease and stroke by offering healthy behavior support services to
women aged 35 to 64. Participants in this program are navigated to WISEWOMAN
resources via their enrollment in the Screening for Life program.
The Delaware Emergency Medical Diabetes Fund provides diabetes services,
medications, and supplies to residents of Delaware on an emergency need basis. It
provides a maximum payment of $500 per client, per 12 consecutive months, for items
directly related to diabetes that will eliminate or alleviate the medical condition. The fund
provides payment for items directly related to diabetes that will eliminate or alleviate
emergency conditions.
B. Project Goals
DATA MANAGEMENT: This front-end data collection system is used to support DPH's
response to statewide strategies aimed at mitigating the impact of chronic disease risk

factors affiliated with cancer, hypertension, diabetes asthma and other chronic
conditions. This database works simultaneously with Smartsheet's data collection tool to
collect specific points of data relating to all BCPC and BCD programs. The goal of this
RFP is to integrate these two data collection and management tools. This will align
federal grant requirements, internal state operations with external clinical operations with
SFL clients, comprehensive cancer program providers and health systems which will
allow for ease of data collection, production of federal/state reports and establish a more
robust centralized hub to systematically support decision making.
BCPC and BCD program operations under this RFP will require cross-collaborations
between all programs and successful applications will propose evidence, practice, or
research-based strategies that support expanding inclusivity and contribute to more
racially equitable outcomes to ensure sustained interventions that achieve Delaware's
goals for Quadruple Aim - improving the health of Delawareans, enhancing the patient
experience of care, reducing health care costs and improving provider satisfaction. This
RFP program is DPH's opportunity to strategize about how to work across sectors,
address the long-term health and social needs of a community and remove structural
barriers to health for socially vulnerable populations in Delaware.
Please Note: The use of the terms "bidders," "vendors," "contractors," and "applicants"
are used interchangeably when describing the activities outlined in this RFP. Likewise,
the use of "shall", "must" and "will" are used interchangeably when describing
applicant/bidder duties.
Eligible Applicant Requirements
Bidders should critically evaluate if their organization is able to meet the organizational
capacity expectations, can fully commit to carrying out expected statewide activities
outlined within each Component area of the RFP applied for, and can meet deliverable
and reporting expectations before preparing a proposal for funding. The state is seeking
organization(s) to support the BCPC and BCD in providing expertise in planning, design
development and implementation of activities to ensure DPH maintains federal and state
compliance for operating this complex statewide initiative.
The awarded vendor will:
- Provide turnkey system updates and sustainability services for the Cancer and Chronic
Disease Data Management system (CCDDMS) formerly known as the Screening for
Life (SFL) database.
- Maintain and support ongoing data conversion and enhancements across supported
programs.
- Generate required reports monthly and/or upon request.
- Develop queries for often viewed reports based on selected date range.
- Data exports must be delivered in a meaningful, useable format ready for program use
and visualization.
- Create a data visualization dashboard within the CCDDMS to facilitate program use
and information dissemination.

- Provide ongoing support for the HCC/SFL application process, including but not limited
to compliance reporting for CDC's NBCCEDP and WISEWOMAN programs.
- Host the database securely for the length of the contract.
- Integrate and maintain a patient navigation process aligned with CDC's NBCCEDP
grant deliverables.
- Create and maintain data dashboards for, but not limited to:
* Chronic Disease Self-Management Programs (CDSMP)
* Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHABPSM)
* Emergency Medical Diabetes Fund (EMDF)
* Patient Navigation
* B&C BARS data collection and other clinic-level cancer screening information
- Develop and maintain a pharmacy claims reimbursement process for EMDF
participating pharmacies.
- Facilitate electronic submission of claims through EDI, EPIC, or other secure health
information exchange platforms.
- Establish and maintain a Smartsheet license to support, but not limited to:
* Delaware Cancer Treatment Program (DCTP) application tracking
* WISEWOMAN program data collection and tracking
Awarded vendor(s) must collaborate and work with the current Smartsheet
vendor to transfer current data sheets and maintain the integrity of historical data
collected under the current Smartsheet vendor contract for patient navigation,
practice transformation, evidence-based interventions, Delaware Cancer
Treatment Program, and all other landing pages and data collection pages for the
DPH programs under this RFP.
- Patient navigation data collection
- Health Promotion Advocate (HPA) data collection and HPA Health Information Report

- Practice transformation data collection for CDC B&C BARS reporting
- In the event of a change to the current data collection and reporting process between
the Delaware Cancer Registry and the National Program of Cancer Registries via the
CDC Registry Plus suite, vendor(s) may also provide data collection, case abstraction,
data transfer, and ongoing maintenance associated with all activities related to the
Delaware Cancer Registry. All vendors should address in the proposal the level of
capacity to provide such services which may include natural language processing of
case records received, data visualization of collected information, contacting
mandatory reporters to confirm data submissions, and troubleshooting issues related
to the DCR and its link to the CDC Registry Plus and related systems.
-Provide at the database logon screen an option for users to choose to display a
Spanish language version of the application and ensure all components are accurately

translated and updated as changes are made to the enrollment screen. Other
languages may also be offered.
C. SCOPE OF SERVICES
Please refer to Appendix B for additional details/requirements for the Scope of Work.
All components listed in this section and Appendix B are mandatory.
III. Required Information
The following information shall be provided in each proposal in the order listed below. Failure
to respond to any request for information within this proposal may result in rejection of the
proposal at the sole discretion of the State.
A. Minimum Requirements
1. Provide Delaware license(s) and/or certification(s) necessary to perform services as
identified in the scope of work.
Prior to the execution of an award document, the successful Vendor shall either furnish
the Agency with proof of State of Delaware Business Licensure or initiate the process
of application where required.
2. Vendor shall provide responses to the Request for Proposal (RFP) scope of work and
clearly identify capabilities as presented in the General Evaluation Requirements
below.
3. Complete all appropriate attachments and forms as identified within the RFP.
4. Proof of insurance and amount of insurance shall be furnished to the Agency prior to
the start of the contract period and shall be no less than as identified in the bid
solicitation, Section V, Item G, subsection 8 (Insurance).
