ADDENDUM NO. 2

ADDENDUM NO. 1 issued to revise the term of agreement provision

The City requests proposals from qualified and reliable insurance carriers and administrators for the following:

1. Self-Insured Dental Plan Network and Administrative Services
2. Health Savings Accounts
3. Flexible Spending Accounts including Dependent Care
4. Employee Assistance Program

The successful Vendor must meet all requirements of this RFP, maintain proper licensing, and comply with all federal, state, and local laws and mandates relative to the services specified in this RFP.

ATTACHMENTS

Attachment A – HUB will release Attachment A directly to carriers. Attachment A will include the census, rates, plan designs, reporting, etc. To request Attachment A, contact Caroline Smith via email, caroline.smith@hubinternational.com .

Sealed proposals may be electronically submitted through the City E-bid system accessible via City’s web site at https://gtowntx.ionwave.net/Login.aspx on or before April 24, 2023 at 2:00 PM and immediately thereafter publicly opened and read aloud.
