ELEVATOR MAINTENANCE AND RELATED SERVICES

Project Description: Provide elevator maintenance and related services for the listed vertical
transportation systems, per the attached RFQ No. 113859, Attachment A - Elevator
Equipment, Attachment B - Elevator Full Service Maintenance Agreement, and Attachment
C - Site Map.

Quote shall include all material and non-material costs, labor, labor-supervision,
travel, applicable fees and taxes (federal and state), transportation costs,
shipping, warranty, administrative support costs, overhead, expediting fees,
equipment, tools, parts, supplies, insurance premiums, and any incidental and
miscellaneous costs to complete the scope of services.

Special Instructions

1. See attached RFQ No. 113859, Attachment A, B and C for detailed instructions.

2. Direct all technical questions concerning the scope of services in writing to Daryl

3. Offerors may visit the project sites on any workday from 8:00 a.m. - 4:30 p.m. There
is no charge for parking. Contact Daryl Masanda to arrange a site visit: (808) 932-8814.

4. To be eligible for award, Offeror must be compliant with the Internal Revenue Service,
State of Hawaii Department of Taxation, Department of Commerce and Consumer Affairs, and
Department of Labor and Industrial Relations. See attached RFQ No. 113859 for additional
details.

5. The attached Quotation Breakdown Form must be completed and
submitted as an attachment to the SuperQuote online submission by the quote submission
deadline.
