The County of Placer is soliciting proposals from qualified firms or groups to provide individual needs assessments, education, enrollment and/or retention assistance for eligible Placer County residents, and support continuity of coverage and access to equitable, coordinated care through Medi-Cal. This assistance in reaching out to the uninsured is vital in helping Placer County to become an even healthier county. This RFP will result in the award of contracts to one or more firms or groups. Placer County’s Department of Health & Human Services (HHS) has allocated $442,000 for these contracts, which will be effective for an initial term of approximately fourteen (14) months. The contracts may be renewed at the County’s option for two additional periods, provided sufficient funding is available.
