Scope of Work: Contractor shall perform debrushing and restoration on the Durnell drain beginning @ STA 128+60 (East of Lincoln Parkway LLC property at 6927 Lincoln Way), following the drain westerly to STA 143+50 (east side of Hadley Rd). See map for location. Total Length = 1,490 L.F. Working side is north side of drain Debrush 30' from top of bank on working side and 10' from top of bank on non-working side. Spoil must be hauled off-site Access shall be the parking lots of Master Spas. Contractor will need to coordinate with Master Spas and FW Pools with a schedule. See attached documents for project details

NOTE: Contractor must follow IC 36-9-27, applicable state/local laws, and Allen County Surveryor's Office Technical Specifications. Contractor shall utilize the regulated drain easement. Any access and damage outside the easement is the responsibility of the Contractor. All drain work is subject to Allen County Surveyor's Office approval. 48 hours notification for inspection is required before work can begin.

Scope of Work: Contractor shall perform Tree removal and Restoration on the Drain. All
Work shall be in accordance with the Allen County Surveyor's O(cid:431)ice Drain Maintenance

Supplemental Work not Covered in Technical Specs: None
Variation from Technical Specs: Contractor shall coordinate all work with property
owners. Debrush 30' from top of bank on the working side, and 10' from top of bank on the
non-working side.
Limits of Work: Work shall begin at STA 128+60 and proceed to STA 143+50.
Access: Access shall be the parking lots of Master Spas. Contractor will need to

Contract Timeframe: Work must be completed within 90 days, weather permitting.
Provide 48-hour notice to ACSO prior to start for inspection and approval.
