The Tarrant County Hospital District d/b/a JPS Health Network (the “District”) is seeking proposals for the provision of Digital Forms Management Solution.

The District is soliciting vendor proposals from vendors capable of supplying the District with a Digital Forms Management Solution (the “Service(s)”), as set forth and specified herein (See Section II, Business Requirements , attached hereto and incorporated herein for all purposes). All Solicitation Responses must be delivered to the District by the date and time, and in the manner specified in Section I.B hereof to be considered a Solicitation Response by the District. It is the sole responsibility of the vendor submitting a Solicitation Response (“Respondent”) to ensure that its Solicitation Response is delivered to the proper location on time and in the manner set forth herein.

The District will reject any proposal that fails to comply in all respects with the instructions set forth herein for responding to this Solicitation. NO EXCEPTIONS WILL BE MADE .
